  • Motor unit = a nerve cell (neuron)+ muscle fibers
  • In PPS there is degeneration of individual nerve terminals in the motor units

    • 1) Poliovirus attacks neurons in brainstem and spinal cord.

    2) Surviving neurons create new terminals to supply orphaned muscle fibers.

    3) This results in recovery of some muscle movement and enlarged motor units

    4) Years-decades of excessive use of enlarged motor units adds stress to neurons.

    5) Neuron fails to maintain metabolic demands of all the new pathways, resulting in motor units slowly deteriorating.

    6) Eventually, nerve terminals malfunction and permanent weakness in any skeletal muscles occur.

    • No evidence on the efficacy of medications on improving fatigue.
