If you live in Toronto and have a postal code that starts with the letter "M", you will have to start the police check application after September 1st, 2022. 

2. The preplacement coordinator, Suzette Martinuzzi, will send you the Toronto Police application form to your email account. Once you received it, you must download it and print it on a legal size (8.5x14) paper ONLY. No other format or size is accepted. 

3. You must fill out and complete all the box sections from top to bottom, making sure that your writing is clear and legible and that you sign your application form.

4. You then must obtain either a bank draft or money order payment of $20.00 dollars and make it payable to Toronto Police Service. No other payment method is accepted. Keep your bank receipts as your proof of payment. 

5. You must submit your Toronto Police application form, payment and a photocopy of government issued photo ID (either a Driver’s Licence or passport or Ontario ID purple card) by registered mail at their business address below: 
Toronto Police Headquarters Records Management Service, 4th Floor 40 College Street, Toronto, ON M5G 2J3

6. The application form will take 4-6 weeks or longer to process. Once it is processed, your police check record will be directly mailed to your home address. Due to the amount of time this process can take, you will have ensure your start and complete this process before the Paramed submissions deadline which is December 16, 2022. You are required to complete the police check by this date in order to complete a field placement in Fall (September) of 2022 in the CYC program.

7. If you do not receive your police check record after 4 weeks, you must send an email to Toronto Police Service directly at referencecheckprogram@torontopolice.on.ca with you full name, date of birth, address, and the date you send it to them. 

Additional notes: 
It is your responsibility to make sure that you receive a clear police check record prior to your program-specific deadline. Otherwise, you will be excluded from clinical/field/dental placement which may jeopardize your academic status in the program
