Heroin can also be smoked which causes breathing problems. after someone injects or snorts heroin, they typically experience an initial euphoric(an affective state in which a person experiences pleasure or excitement and intense feelings of well-being and happiness) rush that may be accompanied by nausea and vomiting and flushing of the skin. That was the short term effects which usually last between 3-5 hours. The long term effects are damaged and collapsed veins, cuts and scabs from picking at itchy skin, loss of appetite, stomach cramps, diarrhea, weight loss, respiratory wheeze, fluctuating moods and depression and many more. Other symptoms are itching,  vomiting, constricted pupils, impaired mental functioning, slurred, incoherent or slow speech, disorientation and clumsiness.

There is this thing called being "on the nod" which is where someone will seem very alert one minute and the next he will be going in and out of consciousness. They experience slow breathing, lower pulse, and may look like they are about to fall down but they usually don't. Heroin overdoses killed more than 13000 people in 2016. signs of a heroin over dose is slow or shallow breathing or no breathing at all, pinpoint pupils and cold clammy skin.

It might be other drugs but heroin is definitely a possibility.
