Depending on how you see the situation, smoking a cigarette could be a sacrifice or not, from my view I see it as sacrificing her well-being just for her to prove to herself and her mother that she isn’t a kid anymore. To sum it up, she’s standing up to her mom’s strictness and trying to prove that she isn’t a kid anymore, and she’s sacrificing her health for it.

In later pages, it explains how she lies for the sake of her and her family's safety and she has to be very strict and overprotective of Marjane because of how dangerous Iran can be. So she stands up for what she wants, which is what's safe for her and her family, and has to sacrifice a good relationship with Marjane so that she stays alive.
