The CNC (Computer Numerical Control) machine is one of the most important machines in the Mechatronics program that I’m currently studying. It features high accuracy, programmable control, and the ability to automate complex tasks. Its main functions include cutting, drilling, milling, and shaping materials such as metal, plastic, or wood.

To operate the CNC machine properly, there are four essential steps. First, you must clean the work surface and secure the material in place. Second, upload the design program and verify tool paths. Third, start the machine and monitor it to ensure everything runs smoothly. Finally, after completion, remove the finished piece and check it for errors or sharp edges.

Each step must be carried out carefully to ensure safety, precision, and product quality.

At the time of the incident, Mr. Bach was using a portable ladder to arrange boxes on an upper storage shelf. During the process, one of the boxes was accidentally dislodged and fell. While attempting to catch the falling box, he lost his balance and slipped off the ladder, resulting in a minor injury to his right hand. Immediate first aid was provided, and he was sent to the hospital right away for further examination.

The equipment involved in the incident included the ladder, the box, and the storage shelf. According to the investigation, the ladder was found to be not in a good condition. It was slightly unstable because of a missing rubber foot, which caused it to slip on the floor. Additionally, the box that fell had been incorrectly placed too close to the edge of the shelf, creating a potential safety hazard during access.

Afterall, i dicided that the main causes of the incident were identified as both equipment malfunction and improper storage practices. The absence of a rubber foot on the ladder significantly reduced its grip on the floor surface, and the improper placement of the box increased the risk of falling objects.

To prevent similar incidents in the future, the following recommendations are proposed:

1. Conduct regular inspections and maintenance of warehouse equipment, particularly ladders and lifting tools, to ensure operational safety.

2. Arrange storage boxes in a balanced and secure manner, with heavier items stored at lower levels to minimize the risk of falling.

3. Install clear warning signs indicating load limits and provide safety guidelines in all storage areas.

This report is submitted for internal safety monitoring and corrective action purposes.
