For entries with unacceptable abbreviations, a written confirmation of the intended meaning should be secured before the order is carried out.

Patients should have appropriate alert band for fall.

Fall Prevention Agreement should be duly signed.

Promotional hand hygiene reminders should be displayed in the unit.

Dispenser with alcohol-based hand rub should be present and readily accessible at point of care.

The should be an access to safe and continuous water supply and facilities (soap and paper towels) to perform hand washing.

There should be a list of high-alert medications in every chart.

High-alert medications should be labeled.

High -alert medicines should be stored in high alert bin and properly locked.

Look-alike and Sound-alike Medications in the crash cart should be strategically placed and labelled.

Medication Reconciliation Form should be present in the chart.

Data A should be complete.
