Appearance and behavior; anything that’s noticeable physically that could hint to a problem. For example if their appearance does not match the behavior they present.
Thought processs; how clear and understandable the client is, and weather or not they can communicate. Often times someone with mental problems is unable to complete thoughts, or may jump around a lot from one subject to another in the middle of a sentence.
Mood and affect; does their body language match the mood they’re presenting? Are they saying they’re happy when they look very clearly upset?
Intellectual functioning; how educated are they? What vocabulary do they use? Does it match their age range, or education level?
Sensorium; are they coherent? Are they aware of what’s going on around them? If a client can’t answer at least 3 grounding questions correctly the answer is no. These questions must be simple and something that the client will definitely know the answer to. Such as, their mothers name, what day of the week it is, their name etc.

Formal tests that can be applied to everyone can be very useful, and there are 3 types; projective, subjective, and neurological. Projective tests, like the thematic appreciation test, can be useful but it is possible to have biases when using them. Subjective tests, like IQ, and personality tests, are tests you take yourself. These tests are useful, but only if the person is answering honestly. Neurological tests are brain images that are used to look for possible physical dysfunctions. But there’s tests can sometimes be inaccurate and show problems where none exists or vice versa.
