         <description><![CDATA[<p>The CPMS West team attended the QI Practitioner Programme. From day one, as a team they were committed to what that they wanted to achieve, in that all children referred into CPMS West for an ASD assessment should wait the same length of time be offered an appointment regardless of where they live.  They met regularly as a team and methodically looked at their processes and ideas for change and have completed several PDSA cycle.          <title>Paediatric Occupational Therapy</title>

         <description><![CDATA[<p>I'd like to nominate Anna Moughton, Lydia Doyle and Kate Edmunds from Paediatric Occupational Therapy who started off with a QI Practitioner project looking at the needs of parents of children on the OT waiting list. They engaged with parents and identified the need for a series of videos. They also changed aspects of the waiting process including the telephone hotline information. They then went to the Dragons Den and secured funding for three videos. Scripts were written, actors cast and the videos produced with edits along the way. A lot of work alongside their day jobs- well done to them. </p>]]></description>

         <title>Nominate - Karenza James</title>

         <description><![CDATA[<p>Karenza James is our Infant Feeding Coordinator and works in an integrated role between Portsmouth Health Visiting Team and our Family Hubs as part of our Start for Life Programme. Karenza is quite new to QI work. This year she has completed some amazing servicer user engagement work to develop new service offers is the city. She undertook a survey, face to face consultations and invited service users to be more involved. She has used the service user voice to develop targeted infant feeding clinics where there is an accessible 24 hour self booking system and to create more welcome spaces in our hubs. More recently she has engaged service users in a video to support breastfeeding in the community and is using the service user voice to inform a communications campaign later this year. Karenza ensures she closes the feedback loop to services users, and has plans to evaluate the improvements which include the service user voice. Thanks for taking these brave steps into QI, I am certain it will be an integral part of your work in the future- you are Awesome! She has worked incredibly hard to develop and/or implement a set of services to support perinatal mental health, infant mental health, parent infant relationships and infant feeding.  Charlotte has listened to families feedback to ensure that the services meet the needs of the families and the business.  Charlotte is a skilled leader who works in a relational way with our staff and partners.  The services are an integral part of our family hub delivery model and are supporting the delivery of the healthy child programme.          <description><![CDATA[<p>Wendy has been instrumental in implementing many quality improvement initiatives for our team in the specialist community forensic team. She has transformed our ways of working with inpatient services by creating regular  meetings to discuss care pathways, promote partnership working amongst professionals; with clear goals and objectives. Wendy is also helping to instate an integral assessment process for our patients, that will help clearly direct their care and support needs early on in their care pathway.          <description><![CDATA[<p>Naomi and Jakub are Clinical Managers/Clinicians in Southampton’s Central Community Independence Service.&nbsp; They have worked tirelessly over the past year to improve the Central CIS team’s understanding and management of their demand and capacity. &nbsp;Until this September they hadn’t attended any formal training but had instead met on an ad hoc basis with myself.&nbsp; After each meeting they’ve reflected on the discussions, worked to develop new skills and knowledge through utilising online resources, and applied their learning directly to their service.&nbsp; This is all in addition to their busy day jobs.&nbsp; Throughout this process they’ve shown initiative, dedication and enthusiasm in abundance and it is always a great pleasure to meet with them and hear of their progress.&nbsp; Their tireless work is supporting the service to develop and embed effective and continuous demand and capacity management.&nbsp; At the heart of this is a desire to improve patient outcomes through reducing and managing waiting lists. &nbsp;They have achieved so much and deserve recognition for this outstanding work.</p><p>Nominated by Helen Wharam</p>]]></description>

         <description><![CDATA[<p>Emma completed the Trusts (Countess of Chester Hospital NHS Foundation Trust) Lean for Leaders programme which was designed to provide leaders of the organisation with an introduction to the fundamentals and concepts of Lean methodology and demonstrate their learning by applying the methodology to an organisational relevant project. Emma's project 'Improving KPI for outpatient and discharge prescriptions' delivered great benefits. Emma has since encouraged her entire team to attend different levels of Lean training at the Trust and as a result  her staff are able to, and encouraged to, suggest ideas for improvement and get involved in making them happen including recent projects improving; storage, over production, reducing waste, flexible working to increase workforce retention and labelling standards to reduce errors - well done! You are all great and it is evident how Continuous/Quality Improvement has made a difference for your team. </p>]]></description>

         <title>Nominate - Laura Atherton and the Milk Bank Team </title>

         <description><![CDATA[<p>Laura completed the Trusts (Countess of Chester Hospital NHS Foundation Trust) Lean for Leaders programme which was designed to provide leaders of the organisation with an introduction to the fundamentals and concepts of Lean methodology and demonstrate their learning by applying the methodology to an organisational relevant project. Laura's project. Laura recognised that there was a lack of awareness of the Milk Bank service amongst Community Services and GPs. This can lead to a delay in blood tests being completed, refusal to complete blood tests and negative feedback from donors regarding the process of getting bloods taken. The Milk Bank at Chester is the largest NHS milk bank in England, supplying safe, screened, pasteurised donor milk to vulnerable babies. On average we supply 30 hospitals with 120 litres of donor milk every month. Last year we supported 216 families to donate milk.</p><p>Laura's project aimed to decrease time in number of days taken for blood samples to be received at MBAC to &lt; 30 days by May 2024 and to improve donor experience with regards to blood samples with a target of 90% of donors to agree/strongly agree that ‘The process of getting bloods done was simple’. Results showed donor survey improvement in experience. the number of days to receive blood samples is now 32.98 and continue to be on target to meet the target. Laura also encouraged her team to attend Lean training so they could understand the use of Lean tools and encourage them to be involved in the improvements. The continuous improvement culture can be seen and felt in the Milk Bank - well done team!</p><p><br></p>]]></description>

         <description><![CDATA[<p>Laura (Business Manager, Adult Mental Health) and Louise (Service Manager, Access to Intervention/A2i service) attended cohort 1 of our Demand and Capacity programme.&nbsp; From the first day they have been committed to improving service demand and capacity management with the aim of reducing their waiting lists.&nbsp; They have ensured that the wider team attending the programme has met regularly and worked to identify areas for change and improvements and supported them to review how they use their clinical capacity, undertake job planning for all staff, and review and change staff skill mix.&nbsp; They have ensured that changes and improvements have continued to be made beyond the end of the programme.&nbsp; Their enthusiasm and dedication have meant that demand and capacity management and planning has now become an integral part of their roles: the service is starting to reap the benefits with a reduction in waiting lists.&nbsp; They are currently producing an evaluation of this work.          <description><![CDATA[<p>Team members: Caroline Short, Louise Newman, Penny Caplan, Norma Quirke, Tracy Whitton, Rachel San-Segundo</p><p><br></p><p>This has been an outstanding team in cohort 1 of our Demand and Capacity programme.&nbsp; They have demonstrated exceptional dedication and teamwork. Despite busy jobs, they have held regular, and lengthy, team meetings at which there has always been good attendance and contribution by all team members.&nbsp; They have tracked and monitored actions to which every team member has contributed.&nbsp; They’ve identified areas of change within their sphere of influence and used a driver diagram to focus and structure their change ideas.&nbsp; They’ve undertaken extensive engagement activities with their wider colleagues to support implementation of their change ideas. &nbsp;They have continued to work to this high standard beyond the end of the Demand and Capacity programme.&nbsp; They have also shared their journey, use of the driver diagram, and words of advice with the teams in cohort 2 of our Demand and Capacity programme: the inspirational effects of that presentation were clear to see!          <description><![CDATA[<p>Nelly is one of our community nurses.  During her preceptorship Nelly undertook a QI project on a soft signs assessment. This experience has challenged Nelly to work out new/different approaches to patient care within the constraints of community nursing. Recently, Nelly presented her work at one of the Academy of Research &amp; Improvement celebration events, where she received praise by our chief medical officer. This was completely out of her comfort zone, Nelly should be highly commended for doing this and the work she continues to do.          <description><![CDATA[<p>Emma and Jackie are team leads in community nursing teams. They understood the need for "flow" in their care pathways and reflected on<strong> </strong> the impact of not using dopplers at an early stage as part of lower leg ulcer treatment. They realised that they had patients experiencing long periods on their caseload, this was due to poor healing rates, staff were doing more work and using extra resource. Emma and Jackie then focused on why this was happening and engaged their teams in coming up with solutions; they made sure dopplers were a priority contact; they supported their staff to upskill in using dopplers and applying hosiery. They also made sure it was easy for staff to have the right stock and equipment that was required on visits.</p><p>Consequently, dopplers have become the new norm, and everyone in the team is doing them. Patients now experience quicker healing rates and proactive management of their condition, fantastic for their independence and quality of life. The new process has reduced patients contacts, and staff capacity has significantly been increased within both teams. Excellent work! Nominated by Susan Tatsinkou &nbsp;</p>]]></description>

         <title>Nominate Rachael Meija </title>

         <description><![CDATA[<p>Rachael Meija, Deputy Director of Nursing, realised one of the most important parts of her role was to be out supporting teams, but her capacity was eaten up with reading and dealing with emails. By experimenting ( PDSA) she was able to devise a comprehensive system for managing her emails. She managed to gain a whole working day a week - this was put  back into doing the things that are vital for her role.          <description><![CDATA[<p>The Delirium Steering Group at Liverpool Heart and Chest Hospital (LHCH) is a multidisciplinary team that includes QI.  In September 2022, the Trust recruited to a Mental Health Liaison Psychiatry team.  Since then, the group has reviewed the risk of delirium in accordance with the updated NICE guidelines and have successfully implemented a number of improvements along the patient surgical pathway.</p><p>These improvements have led to reduced delirium-related incidents, more appropriate use of antipsychotics and a better understanding of delirium across LHCH for staff, patients and their families.</p><p>Watch this space for our video to be uploaded.</p><p>Nominated by the LHCH QI Team.</p>]]></description>

         <description><![CDATA[<p>The team were participating in the Improving Physical Health for people with mental illness BTS collaborative - Aim of the ward project was to reduce incidents of Hospital acquired pressure ulcers for later life inpatients by 20% by end of December 2023.   The team began testing Purpose T assessment tool to enable earlier identification and preventative measures to reduce the risk off pressure damage.  The team collected baseline data from Feb 2022 to December 2023.  A number of change ideas were tested on the ward and by December 2023 they had achieved and sustained a 100% reduction in hospital acquired pressure ulcer, therefore surpassing their aim of 20%.  This has been sustained. Big shout out to Ruth Walters, Joanna Pease, Katy McCann and Amelda Atkin.          <description><![CDATA[<p>Over the last year, Rydal Ward have been amazing in making a difference and actively engaging with the Mental Health Act National QI Collaborative. From unearthing root causes of inequity on the ward, to developing meaningful test of changes, they have made a visible difference for patients and staff on the ward. The three main areas they worked on, are: raising Equality, Diversity and Inclusion Awareness, Celebrating Diverse Cultures and created an 'Identity Wheel' to support with planning inclusive and culturally appropriate care. Despite everyday challenges a mental health ward encounters, their commitment to the programme has shined through!</p>]]></description>

         <description><![CDATA[<p>Project team: Joanna Pease, Ruth Walters, Amelda Aitkin, Katy McCann.</p><p>&nbsp;</p><p>The team were participating in the Improving Physical Health for people with mental illness BTS collaborative.&nbsp; They focused on reducing incidents of hospital acquired pressure ulcers.&nbsp; The aim was to reduce HA pressure ulcers by 20% by the end of December 2023.&nbsp; The project commenced in May 2023.&nbsp; One of the change ideas was to improve recognition and timely interventions to prevent pressure ulcers.&nbsp; The team focused on introducing the Purpose T tool as opposed to using the Waterlow and flow chart.&nbsp; &nbsp;They designed their own bespoke training for staff and together drafted a Driver Diagram and measurement plan. Between Feb 2022 and May 2023, they had 8 pressure ulcers.&nbsp; They also introduced a wound care training day for staff and improved accessibility of wound specific nursing care plans to support staff.&nbsp; Body maps were also completed at the point of admission to ensure practice was embedded.&nbsp; Subsequent audits were undertaken as part of a sustainability plan.&nbsp; The results were amazing they had achieved a 100% reduction in pressure ulcers and this has sustained! Well done to the team for this amazing improving safety initiative.</p><p>&nbsp;</p>]]></description>

         <description><![CDATA[<p>Maple Ward participated in the GMMH Reducing Restrictive Practice Collaborative during the past year and their engagement throughout the process has been outstanding! Maple's Ward vision was to improve the quality of support provided to patients, mainly diagnosed with an advanced organic condition, and reduce the amount of time spent under enhanced observations. They implemented 3 key change ideas, which compliment and inform each other: Daily Safety Huddles, Traffic Light Support Plan and, Interest Checklist/ Tips for Engagement. Their work, not only contributed to the reduction of restraint incidents, but also created a more trauma-informed care environment, improved patient wellbeing and reduced staff burnout.          <description><![CDATA[<p>Juniper Ward have shown great dedication for quality improvement during their participation at the GMMH Reducing Restrictive Practice Collaborative. Aiming to reduce incidents of violence and aggression on the ward and improve patient and staff experience and safety, the project team focused on reduction of observations, introduction of a quiet room and increase of their establishment to facilitate more activities. Through this work, Juniper Ward have noticed a reduction in use of seclusion, a reduction in service users and care complaints and, increased team morale and cohesive working. Patients said that they felt more involved in their care and treatment and had more time  to speak to staff and participate in daily activities. Well done Juniper Ward!</p>]]></description>

         <description><![CDATA[<p>Project team: Ferndale service users, Fiona McKeekan, Keith Whittle, Lacey Thompson, Sarah Marlow, Harriet Duff, Alison Schofield and Lucy Povah</p><p><br></p><p>The project's objective was to collaborate with AFS service users to co-produce a new Care Plan document that would identify patient and carer involvement, admission goals, identified needs, and recovery plans. This was trialled on two wards and has undergone numerous modifications in response to feedback from service users and staff. We are pleased to inform you that the new care plans are now operational on all wards!</p><p>This process has enabled AFS to develop a training package for primary nurses, a new audit for deputy ward managers to conduct in order to evaluate the quality of the care plan against the CQC standard, and a system in which care plans are routinely reviewed during ward rounds to ensure MDT oversight. The subsequent phase of the project involves the development of a training package that will empower service users to take the lead in the development of their own care plans. Following completion of the QI certified leader programme she identified the need to improve the process for temporary patient accommodation. By mapping 3 patients journeys, she was able to identify and escalate the issues, and is currently standardising a new process. This work has had a major impact on financial spend, ensured patients were in the right place for their needs to be meet and has decreased use of temporary accommodation by 50% in her area. But teams were being over whelmed with requests for information, the process was unnecessarily time consuming for staff and appeared to deliver little value for the children. Nicky Barker, School Nurse Team Lead has supported Fiona, by trailing a new process in a SN team which has reduced the demand for information by 80%. So her ward piloted patients attending ward rounds, so they had the opportunity to discuss their treatment plans and ask questions about their medication. She ensure there was an appropriate and therapeutic place to do this.          <description><![CDATA[<p>Yvonne is a Manager for an OPMH Ward (SHFT). She was aware of the research that identified good practice in having visiting times not restricted. However for her staff there was a lot of nerves about how this would work in reality on a ward that had very specific visiting times, so staff could complete their work ( so they thought). Under Yvonne guidance and leadership she took a quality improvement approach, engaging her staff and piloting open visiting times. The results: Families who had to travel long distances found it easier to find times to visit; staff learned from family and carers how to handle specific needs of the patients - enhancing the patient care; families /carers had a chance to be upskilled on new supports the patients needed prior to discharge; and also staff found some of their capacity was increased when family members and carers were there to support/reassure their loved ones.          <description><![CDATA[<p>Emma is the team lead for the North Community team, she completed the QI training last year and has embraced the process and has really engaged the team. She had identified clear problems, and at a team meeting asked the team to look at this large problem and break it down into smaller ones that could be then be explored. Emma engaged the team by dividing the issues and creating working groups where the suggestions for improvement were explored by the working groups, they then came up with the ways to implement the ideas and this has been embraced by all. The results are a smoother process with less clicks from triage to waiting list, enhanced use of the support workers and a team who despite vacancies have managed to control the waiting list.          <description><![CDATA[<p>Katie Ross is a physio working in EHCH and had the initiative to think about how the service could be expanded/enhanced. She completed the QI training and the tools she had learnt from this has supported her to produce a collaborative report on how the service can be expanded to cover more homes in an adjoining patch. It has promoted a workforce skill mix review and has opened opportunities for others at different banding to be onboarded. Katie has worked hard to collect the data and used driver diagrams and process mapping to show the benefits of the change and combined with the understanding of national strategies has produced an impact document that has been presented to the Head of Clinical Services for the division and been signed off to progress.
