         <description><![CDATA[<div>Hi, what is your name and student number? <br>The paraphrasing part (which is marked =20%) is <strong>20 minutes</strong> in length. <br>The literacy screening test is 10 minutes in length (you will write a short 100-120 word paragraph on a general topic you will be given on the day; this part is not given a mark, rather it is recorded as Satisfactory/Unsatisfactory upon completion.          <description><![CDATA[<div>Hi, just wondering if we should paraphrase the information from other sources that are referenced in the texts. eg. the person-centred care text has a definition of empowerment that is referenced (intext citation) should I include this/ paraphrase this ? <br>thanks<br><br>Dear Yasmin,<br>It's up to you which parts of the text you choose to use in your paraphrase. Remember, you only have a word limit of 150 words. This means you cannot include all the information in the paraphrase. Select a few main ideas and then paraphrase them in 150 words. If you have selected information which has a secondary citation as in the example you provided - "definition of empowerment", you don't need to reference this. But YOU MUST PARAPHRASE IT!          <description><![CDATA[<div>Hi, when it talks about a concept/idea and then goes on to give examples, is it integral that we include examples, or is it better to use our words to state and describe the main points?<br><br>Hi Abbey,<br>Make sure to include all the main ideas in your own words. If you wish to also include a few of the examples, you can. For instance, you may wish to include a few examples from the text if you find it difficult to reach 150 words with only the main ideas. However, if you find you are going over the word count of 165 words, you might choose not to include any secondary ideas or examples. Keep in mind, you do not have to include all the examples in the text- maybe just one or two. Do you mean, <strong>Can I include examples from the highlighted extracts?</strong> You can include examples from the highlighted extracts- just make sure you paraphrase the examples into your own words. Note: When paraphrasing, you do not have to change every word from the original text (eg. do NOT change specialised words such as <strong>person-centred</strong>, or <strong>communication,</strong> or <strong>holistic, </strong>or <strong>therapeutic), </strong>rather, try to change the structure and write the paraphrase in your own way/style/words (without changing the author's original meaning).<br>Mandy Salas (PCALS)</div>]]></description>

The book <em>Fundamentals of nursing and midwifery: A person centred approach to care </em>is an <strong>Edited Book</strong> - see p.11 APA 7 guide (ie. the authors on the front cover are the editors. We can see they are the editors as if we go to the Title page inside the cover of the book, it says something similar to: ..This edition is edited by. Also, if you check in the Contents page of this textbook, there are no different authors listed for every chapter). The book <em>Critical conversations for patient safety </em>should be referenced as a Chapter or Article in an Edited Book - see p.12 APA 7 guide (We can see it is an edited book as on the front cover it says Edited by.. Also, it is a Chapter in an edited book because if you check in the Contents page of this textbook, there are different authors listed for every chapter.) <strong>HINT: </strong>On your highlighted extract copies from Assessment 1, if you look at the bottom of all 3 extract pages, the full reference list entry is written correctly there!! :) For in-text citations for the extracts, please check the APA 7 guide- under the examples for how to do the reference list entries, it gives examples for how to do in-text citations. Note: for an Edited Book, use the editor name/s in the in-text reference. But, for a Chapter or article in an Edited Book, use the Chapter author's name/s in the in-text reference. <strong>TIP: </strong>for 3 or more authors you write the <strong>first </strong>author's <strong>surname</strong> + <strong>et al.</strong> (with a full stop after al<strong>.</strong> )<br>eg. According to Hill et al. (2017), ...   or/ ........ To answer your question, it sounds like you are trying to reference a <strong>secondary source</strong> (see p. 8 in the APA 7 guide), and that the source you have read is a <strong>Chapter or article in an edited book</strong> (see p. 12 APA 7 guide). For a Chapter in an edited book, you begin your reference list entry with the Chapter author, and your in-text citation is the <strong>Chapter author</strong> (not the Editor of the book). In this case, your Chapter author is the secondary source (B) which has referred to the Primary source (A) within their Chapter. The way to show this in an in- text citation is as follows: eg.<br>According to Author A (year A , as cited in Author B, year B), ..<em>insert paraphrase of author A's idea</em>.      or/<br><em>Insert paraphrase of author A's idea and put the citation all in brackets at the end</em>... (Author A, year A, as cited in Author B, year B). To give an example of referencing a secondary source from extract 1: <br>Chenoweth et al. Eg. If your body 3 is about documentation, begin this paragraph with a topic sentence introducing the main idea of documentation and its importance as part of professional communication. Support this main idea (documentation) throughout your paragraph with explanations, and support your ideas with evidence (sources) that shows how proper documentation as a form of professional communication <strong>is critical to promote therapeutic nurse-patient relationships and patient safety </strong>(your thesis).  Supporting ideas could possibly include ideas based around questions you ask yourself: <strong>WHAT? WHO? WHEN? WHERE? HOW? WHY? WHAT IF? </strong>If you ask yourself these types of questions in every assessment task you do, and search for the answers in scholarly sources, this can give you supporting ideas for your body paragraphs, then it is up to you to decide which are the most important supporting ideas to include to answer your essay question, and how many supporting ideas can fit within your limited word count. Possible questions you could investigate are:  <strong>what </strong>types of documentation are used in nursing, <strong>who</strong> needs to document information, <strong>when</strong> does information need to be documented,  <strong>where</strong> is information documented, <strong>how</strong> should information be documented, <strong>why </strong>does information need to be documented,  <strong>what if </strong>information is not correctly documented. This word limit includes your in-text references, but the word limit does NOT include your reference list.<br>Your marking criteria states that you need to include the 3 set extracts (these 3 extracts come from 2 different textbooks, so for the three extracts you will only have 2 sources in your reference list- Hill, Hall &amp; Glew and Levett-Jones). For the 3 extracts you use, you will need to reference them each time you use them in-text. You also need to include 1 journal article (this can be any journal article that is relevant and current, ie. 2014-2020),  and 1 website (eg. Nursing and Midwifery Board of Australia). You do NOT need to include a reference list entry or in-text citation for the case study scenario you use.<br><br>PCAL staff can give feedback on any nursing or midwifery student's assessment draft for any subject, including this one, however it may be a bit late now to secure an appointment as your assignment is due next Monday 27th April at 11:59pm. First, choose "Align to Left" as your formatting style - From looking at your attached example, I think you may be choosing the "Justify Text" option which lines up your writing equally on both sides of your page- you should NOT choose this "Justify" option- instead choose "Align to Left". Please note, in APA 7, you now do NOT need to type <strong>Retrieved from</strong> for most references (including the NMBA website references) before you include the URL, and your URL link should now be a live hyperlink.<br><br>Type each reference according to the new APA 7 guide, without worrying about spacing or hanging indent when you first type it up- just type all your details in a row and let the computer jump to the next line for you as you type. When you finish typing all your references, please watch the new PCALS video on formatting reference lists now up on the <strong>PCAL vUWS site </strong>under the left side-bar tab: <strong>Library &amp; Referencing</strong>. So each main concept used to support your thesis is given one body paragraph each. Please do not include all 3 concepts in one paragraph, as this paragraph will end up being too long. In terms of word count for this essay which is 100 words total (+/- 10%), essay Introduction and Conclusion paragraphs are normally 10% of the essay word count each, so 100 words <strong>each</strong> for the Introduction and Conclusion paragraphs. That leaves you 800 words for the body paragraphs, giving you about 250-300 words per body paragraph if you are including 3 body paragraphs.<br><br>In terms of the extracts, you are expected to paraphrase at least one thing from within each of the highlighted parts of each extract.          <description><![CDATA[<div>Hi <br>Do we only introduce one concept for each paragraph for example effective documentation for paragraph one or can we introduce more than one?<br>thank you<br>Hi, the structure of the essay body in terms of order of concepts is up to you- just make sure it makes sense and flows well, and you have integrated your thesis throughout your body (ie. that nurses need effective nurse-patient communication to promote therapeutic relationships and patient safety.) You could do one body paragraph each for each of the 3 concepts (verbal and non-verbal communication, person-centred care, and documentation). Or perhaps you could integrate verbal communication with documentation (as documentation is a form of verbal communication). Please keep in mind, most body paragraphs tend to focus on one main concept, but if you choose to integrate two concepts in the same body paragraph, make that clear in that body paragraph's topic sentence. Your topic sentence, ie. the first (or sometimes second) sentence of each body paragraph, is where you need to introduce what that paragraph is about.<br>Whichever order of concepts you choose in your body, ensure that you reflect the same order in your Introduction's <strong>outline</strong> sentence (the last sentence of your introduction)- here you should preview your body's main concepts in the SAME order that you will discuss them in your body.<br>Mandy Salas (PCALS)<br><br></div>]]></description>

First I watched the webinar for the essay and i was confident that i knew how to reference the extracts, however, after watching the referencing video recording at the bottom of the Assessment 2 tab, i got extremely confused as the webinar and that video had completely two different ways of referencing for the same book/extract. I already did the referencing the way the webinar explained it, but do i have to completely change everything? <br>Hi Melissa,<br>apologies for any confusion caused. Your unit coordinator has said that both methods are acceptable in this essay, and you will not be penalised in terms of marking for using either method. So if you have done it as a Chapter in an edited book, you can leave it as is if you wish.<br>The reason I suggested you can reference this book simply as an Edited book, is because it is not clear from the 3rd Australian and New Zealand ed. <strong>Contents</strong> page who the author is (no authors are listed on the Contents page), and in the Contributors and reviewers page of this book, it is difficult to figure out who the primary author of this chapter is, as 2 separate authors are listed for this chapter, ie. Glew and Hill- it is not clear who should be listed first in your in-text &amp; reference list citations, as authors of a text are often listed in terms of who contributed more, rather than in alphabetical order. The easiest way to reference this book is simply as an Edited book, so the author for both extracts 1 and 2 will be Hill et al. (2017). In this case, this text will only be listed once in your reference list as an Edited book, but will be used at least twice in your body (once for extract 1, and once for extract 2). (2017) book, you can reference it as an Edited book, so the author with be the Editors. But, they will be referenced twice, as they are different chapters written by  different chapter writers. Then the Levett-Jones reference is another reference. So that totals 3 references. You also inlcude the NMBA and one journal article. <br>Suzanne D'Souza (PCALS).<br><br>Dear Amy,<br>there seems to be some miscommunication about how you can reference the Hill et al. book. If you do it as an Edited book, you will not have individual chapter authors, so while you'll use this text at least twice in your body (once for extract 1, and once for extract 2), you need only reference it ONCE as an edited book in your reference list (so a minimum of 4 sources in your reference list would be ok- the 2 textbooks the extracts came from, plus a website plus a journal article). If you've referenced it as a Chapter in Edited book, then you'll have 2 different sources in your reference list (so a minimum of 5 sources in your reference list). I also wanted to know if i can use a pain scale from 1-10 to chart it down in the assessment part of the health video? </div><div>Kind regards, Shaman<br>Hi Shaman,<br>the total word count for the Clinical note (including the Patient progress notes found at the bottom of p.2) is 500 words, with 10 marks total, the marks allocated as indicated on the Clinical note. Roughly, I would say this equates to about 50 words per 1 mark, so write about 50 words for the Recommendation part (worth 1 mark) and about 200 words for the Patient progress notes part (worth 4 marks). For the recommendation, you could try to write it all in one sentence, eg.  <em>Patient's name </em>is recommended to 1, 2, 3 and 4.(<em>substitute 1, 2, 3 etc with a summary of each recommendation- Note: there is no need to number the recommendations</em>). For the Patient progress notes, write one continuous paragraph in which you summarise the most important information from your notetaking above in your Clinical notes on page 1 and 2 (ie. the notes you made during your interview). However, I have just seen in the FAQ's thats been recently uploaded that it should be 7-10 minutes. I have already recorded my video with a friend. <br><br>Also with the template do we remove the lines to type our answer?<br><br>Hi Tiarna,<br>the FAQ questions now state 9-11min long is ok. Regarding the lines, I understand that the lines move along the page as you type, making your clinical note possibly extend over 2 pages (and the FAQs state to keep it to 2 pages in length). The key to the assignment is to show your assessor that you can conduct a safe and professional patient interview, making sure you ask all the key questions about allergies, stress, diet, etc. The patient sheet can't possibky give all the information that you might need [otherwise it would be pages long, wound't it], so on occasion you need to improvise. <br>I hope that helps, good luck!</div>]]></description>

That means it will be really hard to put it in the sequential order of clinical notes. The key is to practice it many times by yourself and get used to asking the questions and filling in the notes section [most probably in a different order]. I can see in your observations that your blood pressure is... Do you know if this is normal for you? George's answer= No, I don't know. Then perhaps follow this up with your recommendation for a review with the doctor. Or if you think it could be linked with his diet or that he was a long term smoker you could perhaps mention that briefly, but then still follow up with the recommendation you suggested.          <description><![CDATA[<div>Hi, I was wondering if i have to follow the ISBAR template when I am conducting the interview?<br><br>Hi, what's your name and student number? It can be useful to follow the order of the template on the Clinical Note (which follows the ISBAR format) so that you do not get lost, nor miss out on any important information. In saying this, your assessment is a conversation, so it may be more natural to speak about things in a different order, eg, give your recommendations as you go along, rather than waiting until the end to give your recommendations, eg. after they tell you about their regular diet, you could link this back to the Observations if you feel the diet is causing something to be out of normal range, and then make a recommendation about diet before moving on to ask a question about alcohol intake. (Either option is acceptable, ie. recommending as you go along, or recommending at the end. Though if you recommend at the end, you will need to recap what the patient told you earlier in the assessment before giving each recommendation, eg. you said that... ). Please remember, you need to make notes while you are conducting the assessment/during filming (as you would be doing in real life), so if you do go out of order from the Clinical note format, make sure you are writing the information in the correct place on your Clinical note -eg.  you may need to flip your page over to p.2 while you're talking, make your notes, then flip back to pg. 1.<br>Mandy Salas (PCALS)</div>]]></description>

I would have to sit really far away from my computer to get the paper consistently. So does that mean our name, teacher's name, date of assessment also contribute to the word count? If yes then I dont think so it will be fair because I have four words name and I have to write it on top, bottom and do the signature as well which increases the word count without the proper information regarding the assessment. I am still not sure but shouldnt it be from the actual writing such as from introduction. This assessment does have higher similarity in comparison to other assessments because you are all using the same case studies.
