I think that the doctor is called 'Doctor Death' because this doctor comes to find out wheather or not people are dead or to tell people how long they have left to live. My evidence is 'He was thilthy and pale and dried out and I thought he was dead.' He also may be a bit obsessed with the subject with death.

It shows that the garage is rusty and broken because 'tugged' shows that the door is hard to open which leaves me to believe that the door is covered in rust. Also they are not taking care of the garage which suggests that it already is broken and collapsing.
