         <description><![CDATA[<div>Tiger Initiative -2005</div><div>The TIGER Initiative is focused on helping the nursing profession to adopt informatics tools, principles, theories, and practices that make healthcare safer and more effective, efficient, patient-centered, and equitable for all stakeholders.</div><div>Nursing professionals have joined the TIGER Initiative to collaborate on nine important topics. These nine topics include:</div><ol><li>Standards and Interoperability</li><li>Healthcare Information Technology National Agenda/Policy</li><li>Informatics Competencies</li><li>Education and Faculty Development</li><li>Staff Development/Continuing Education</li><li>Usability/Clinical Application Design</li><li>Virtual Demonstration Center</li><li>Leadership Development</li><li>Consumer Empowerment/Personal Health Record</li></ol><div>Each collaborative team is lead by an industry expert on a specific topic.          <description><![CDATA[<ul><li>D stands for Data</li><li>I stands for Information</li><li>K stands for Knowledge</li><li>W stands for Wisdom</li></ul><div><br>Data is a collection of facts in a raw or unorganized form such as numbers or characters. Without context, data can mean little. Information is data that has been “cleaned” of errors and further processed to make it easier to measure, visualize and analyze for a specific purpose. When we understand information and how to apply it to achieve our goals, we turn it into knowledge.          <description><![CDATA[<div>The American Association of Critical Care Nurses defines alarm fatigue as a sensory overload that occurs when clinicians are exposed to excessive alarms, which can result in desensitization to alarm sounds and an increased rate of missed alarms.&nbsp;<br><br>Factors that contribute to alarm fatigue:<br><br></div><ul><li>Alarm parameter thresholds were set too tight</li><li>Alarm settings not adjusted to the individual patient’s needs</li><li>Poor EKG electrode practices resulting in frequent false alarms</li><li>Inability of staff to hear alarms or detect where an alarm is coming from</li><li>Inadequate staff training on monitors and alarms</li><li>Inadequate staff response to alarms</li><li>Alarm Malfunction</li></ul><div>&nbsp;Tips to combat alarm fatigue:<br><br></div><ul><li>Have an alarm-management process in place.</li><li>Review and adjust default parameter settings and ensure appropriate settings for different clinical areas.</li><li>Determine where and when alarms are not clinically significant and may not be needed.</li><li>Create procedures that allow staff to customize alarms based on the individual patient’s condition.</li><li>Make sure all equipment is maintained properly.</li><li>Organize an interprofessional alarm management team.</li><li>Develop policies/procedures for monitoring only those patients with clinical indications for monitoring.</li><li>Develop unit-specific default parameters and alarm management policies.</li><li>Provide ongoing education on monitoring systems and alarm management for unit staff.</li></ul><div>Reference:<br>Gaines, K.
