Right before this scene, Haku and Chihiro just crossed the bridge and chihiro couldn't hold her breath, so they hid in the bush, and then Haku told her to run down the stairs, and go somewhere.

Haku crashed into the room, injured. Chihiro fed Haku the medicine ball, Haku then spat out the golden seal along with the worm hanging onto it.
