When your line manager has digitally signed off your portfolio, you will be automatically marked as completed. You will receive a confirmation email from us, but please be aware that this might not be immediate.


In the rare circumstance that you have completed your portfolio either by hand or PDF, please take a screenshot or a photo of the "Line Manager Sign Off" page and send it as an attachment to leedsth-tr.professionaldevelopmentteam@nhs.net.

We will respond within 48 hours with an acknowledgement that this has been received.
