         <description><![CDATA[<div>This is a comprehensive account, but my take home messages are:<br>•Can have wide differentiation including neurology referral or MRI</div><div>•Onset is typically within 2–3 days of the dural puncture, but can occasionally be much sooner or later</div><div>•Symptoms are classically a searing frontal or occipital headache, sometimes exacerbated by head movement / upright posture, and relieved by lying down</div><div>•Symptoms can, however, be confusingly nebulous eg neck stiffness, nausea, vomiting, hearing loss, tinnitus, vertigo, paraesthesia and visual disturbance.&nbsp;</div><div>Therapy:<br>•Get an expert in, someone who sees them often, normally an obs anaes:</div><div>•maximum analgesia, typically paracetamol, an NSAID and a weak opioid.</div><div>•Failure of this regimen should prompt consideration of an epidural blood patch.</div>]]></description>

         <description><![CDATA[<div>Apart from adrenaline which is pre-mixed, at best modest evidence published for the adjuncts used in practice. NB Caution mixing drugs . Its normally unlicensed practice, and an opportunity for potentially catastrophic drug error.<br><br>Dexamethasone 8mg added to brachial plexus blocks “shown” to prolong block significantly. However, likely publication bias and iv dexamethasone has been shown to have similar effects to interscalene administration.<br><br>Clonidine: neutral published evidence for peripheral nerve blocks.<br><br>Spinal clonidine: less is more. It is likely that the ceiling dose for prolonging the block is less than the ceiling dose for side effects (mainly hypotension). So higher doses cause more side effects than lower doses, without extra benefits. 15-30 mcg intrathecally reasonable practice.<br><br>Opioids: more evidence of safety and benefit. Again, less is more. No evidence of one opioid over another for safety, but fentanyl/diamorphine/morphine last 6/12/24 hours respectively. Local practice is preservative free morphine. Again less is more: significant nausea/itching (and presumably respiratory failure) at doses higher than 150mcg, with no apparent increase in analgesic effect. 100-150 mcg intrathecally reasonable practice.<br><br><br><br></div>]]></description>

         <description><![CDATA[<div>This approach offers a useful window in patients who are unable to position themselves easily or have abnormal anatomy (e.g. kyphoscoliosis). Needle entry is slightly caudal and lateral to the inferior aspect of the spinous process of the chosen interspace. After insertion in a cephalomedial direction, the needle is walked off the lamina until the ligamentum flavum is engaged. Notably, the midline structures of the supraspinous and interspinous ligaments are not involved, and the paraspinous tissue offers little ‘feel’ until this point. The needle on this approach passes through: skin, subcutaneous fat, connective tissue and ligamentum flavum.</div>]]></description>

         <description><![CDATA[<div><br>Growing evidence suggests that ultrasound can be used to identify both vertebral levels and epidural access points more accurately than palpation of surface anatomical landmarks.<br>A scan before the procedure in patients with known altered anatomy (scoliosis, hyperkyphosis, hyperlordosis) or difficult to palpate landmark anatomy (obesity), improves the technical efficiency of the block and reduces the risk of traumatic or failed insertion.<br>Ultrasound may also be used to indicate depth of the epidural space. Trial data suggests that obstetric epidural analgesia is more effective when using ultrasound, compared with surface landmark techniques.<br>Three basic ultrasonographic views can be obtained before needle insertion. These are: ‘paramedian sagittal oblique (interlaminar) view,’ the ‘transverse process view’ and the ‘transverse interlaminar (interspinous) view’. The interlaminar and interspinous views provide an image of the neuraxial structures through acoustic windows.&nbsp;</div>]]></description>
