Introduction
- The Practice Nurse did not introduce herself: her name, her role, her purpose in that consultation

Situation
- She simply listed all the procedures done to Mrs Poole and the results that came from that.
- Used clinical terms that Mrs Poole may not have been familiar with

Background
- Did not touch on her family's medical history. The fact that her husband passed away 6 months prior would have had a huge effect on her own health.
- Her living conditions were not brought up
- Her eating, sleeping and exercise habits were not considered


Assessment
- As mentioned before, the nurse simply listed what was in front of her on paper, and did not conduct any further assessments on Mrs Poole.
- The nurse did not actively seek remedies for Mrs Poole from the information that was disclosed to her


Recommendation
-  When Mrs Poole said that she had not been eating and sleeping well, the nurse just told her that she should bring it up with her GP instead of delving into those issues as a Nurse to help her.

- The nurse did not actively engage in conversation with the patient 
 - Did not seem to show any concern toward's the patient's health
- Use of clinical terminology which the patient may not understand
- If the patient cannot comprehend what information is being delivered, she will not know what questions to ask
- The consultation was rushed 
- The nurse was very confronting 
- Since, the nurse always responded to questions by telling the patient to see the GP, the patient would have felt that there was no use in asking anymore questions if the answer was going to be the same

Mrs Poole was not engaged and open in the conversation. So little relevant information was exchanged. Also, the practice nurse was new to Mrs Poole.
