         <description><![CDATA[<p>We see a need for a centralized transition navigation hub for each DC agency that provides a clear, accurate description of program eligibility, documentation requirements, and services provided by these DC public programs. This would address families duplicating documentation for each agency and also prepare them in an efficient and streamlined way. </p>]]></description>

         <description><![CDATA[<p>Schools need a dedicated transition coordinator, or at least someone knows what programs are out there! Parents should not have to be their own child's transition coordinator and should know more than I do. If you're not informed, your child is out of luck. Make transition services for special ed students as important as offering 40-something AP classes.</p>]]></description>

         <description><![CDATA[<p>DC Special Education Cooperative</p><p>I think the biggest need in Collaborating with DC Schools is to help school leaders truly understand what transition is, the true purpose and intent, and how they can intentionally incorporate transition planning in the student's schedule, curriculum, and planning. Transition cannot be an afterthought. It truly drives the IEP. It is planning that begins with the end</p><p>in mind.</p>]]></description>

Ask the families what they need and how you can assist them. Assign a POC to each school to ensure resources are readily available to families. </p><p><br/></p>]]></description>
