The veterinary nurse should examine the bandage and check if it has slipped, if there's strike through or any swelling above/below the dressing

1- poultice bandages help draw out/ drain abscesses , so has been used to try help the horse with the lameness issue

2- the bandage may not have been tight enough, and it may have gotten too wet

3- try re bandaging the abscess properly, ensuring the site is clean before wrapping to ensure your not harbouring any bacteria

The wound is healing and could be itchy or swelling. There is not enough layers of bandage so could be contaminated.
Remove to assess underneath and reapply.
Assess wound and reapply bandage, advise owners on how to keep the bandage clean etc.

Pain , itchiness , pressure sores could be swollen from the operation.

Assess whether it’s too loose too tight . Check for any strike through. See if there’s enough protective padding around the area to support the limb.

Change the dressing . Could be the wrong type of dressing. Re - assess the wound . Is it infected ? Have the sutures opened up ?

the bandage could be too tight, it might have gotten wet or the wound might be swelling, it could be itchy or irritating them as discharge from the wound might be leaking through
assess for any smells coming from the area, see if there is any swelling above or below the bandage, asses whether the wound is leaking through the bandage, ask the owner if the bandage has gotten wet recently and assess if the bandage is slipping or is too tight
remove the bandage, assess the wound underneath and apply an appropriate dressing and a new bandage

The poultice draws out fluid

May have gotten it wet and the moisture has gone through all the layers. The hoof also may not have been dried off properly after soaking.

Potential box rest to prevent it getting wet, or a different type of bandage, soaking the hoof again, drying it properly and then applying another poultice.

Could be swelling, pressure sores, most likely to be being crate rested 3 days post cruciate surgery so probably very bored as well! Licking is often a sign of pain/a self soothing measure
Assess to make sure there is no swelling or strike-through, that the licking has not damaged the bandage at all.
Change the bandage, make sure that the dog is being given enough pain medication and has sufficient mental stimulation.

Abscess might be at a different healing stage where it is more suited that the bandage be applied dry rather than wet to help absorb all the moisture.

The purpose of the poultice bandage is to draw out the abscess from the hoof. The horse is still lame - will take a while to be completely sound but suggests the poultice is not working properly and the abscess is still causing pain. The poultice may have not been tight or secure enough Which is why it has slipped off. The owner also says it is wet, is this from the damp poultice and possibly too wet or is the horse standing in a wet stable or field.

Maybe a nurse needs to go out to the horse to assess the competency of the owner at applying the bandage and check the horse is in an environment suitable for it during poulticing (dry clean stable)

The vet should check that the owner is competent to apply another poultice correctly. The hood should be cleaned for a new poultice to be reapplied

What is the purpose of a poultice bandage: to draw out and absorb exudate from abscess

What might have gone wrong? The owner may not dressed the poultice properly, the horse may have been in a dirty wet stable or field instead of clean dry environment.

The owner may not be changing the poultice frequently enough

What actions should have been taken? The vet should ensure owner is competent at applying poultice, possible send ambulatory nurse to educate owner. Abscess should be flushed clean and redressed with dry poultice with a review of the abscess booked in.

the bandage might be using an adherent dressing like gauze which is sticking in the incised wound, causing discomfort.

check for swelling and tightness as well as damage to the bandage and wetness or contamination.

re bandage the area with a non adherent dressing and make sure it’s not too tight or stuck to fur

Q1 - the bandage could be too tight or causing irritation. The animal might also be experiencing some discomfort as the wound is healing

Q2 - check how tight the bandage is. Check if the bandage has been impacted in any way (chewing, slipping off, has become contaminated)

Q3 - a new bandage may be placed and give advice to owners on distracting the animal whilst the wound is healing. A buster collar may need to be fitted

Type of dressing might be unsuitable for the animal, RVN should remove the bandage as it would now be wet and bacteria could be seeking through the layers, assess the wound once removed, clean the wound, ensure the materials remain sterile and discuss with others whether a different type of bandage should be used or whether the bandage technique on the first bandage could be done better, maybe restricting first time. Then once re bandaged, go over care guide with the owner to prevent animal from intervening.

1) Allergy, pressure, swelling, bandage could be too tight, possible contamination, 3 days worth of itch, moisture…

2) check the tension and tightness of the bandage (one finger should fit), check the skin underneath and assess the dogs pain and discomfort.

3) With a 👍 from the VS the VN should remove and replace the bandage, dry and clean the area, double check wound, ask owner to monitor patients reaction to the bandage.
