         <description><![CDATA[<div>Florence is an 84-year-old lady with hypertension and rheumatoid arthritis. She lives at home on her own, and has weekly visits from her daughter who lives 35 miles away. She also receives daily support from a care worker to help with activities of daily living.<br><br>She is currently prescribed                          bendroflumethiazide 2.5 mg daily<br>lisinopril 10 mg daily<br>simvastatin 40 mg at night<br>methotrexate 20 mg once weekly on a Wednesday<br>folic acid 5 mg daily except on Wednesday<br>paracetamol 1 g four times daily when needed.<br><br>On Friday evening, an on-call GP visits Florence because she has been feeling unwell with symptoms of urinary frequency, dysuria and urge incontinence for a few days. The GP diagnoses a urinary tract infection (UTI) and prescribes Co-amoxiclav 250 mg three times a day. On Sunday, the care worker is very concerned that Florence has deteriorated and is nauseous and confused. She calls an ambulance and Florence is admitted to hospital.<br><br><br></div>]]></description>

         <description><![CDATA[<div>Mrs Scott is 78 years old and has a history of type II diabetes, which is managed with hypoglycaemic medication. Since her husband died two years ago she has lived alone with her two cats, she has been treated with depression.  Mrs Scott fell in her kitchen and was unable to get off the floor, she was found by her neighbour some hours later. <br><br>After being assessed in A&amp;E she was diagnosed with a stable fracture of her pelvis.  Her blood sugar was high.  She was admitted to the ward to continue her care. <br><br>Mrs Scott is 1.6 m (5 ft 3 in) in height and weighs 86kg (13st 7 lb) which gives her a BMI score of 34.  She smokes 15 cigarettes a day.  </div>]]></description>

         <description><![CDATA[<div>Eileen is a 61 yearold woman who has had a long-term diagnosis of COPD. Her medical history also includes anxiety, osteoporosis and pneumonia. She is a retired teacher and has smoked thirty cigarettes a day for almost fifty years. She has no known allergies, and doesn’t drink alcohol or take any recreational drugs. The patient currently lives at home with her partner who’s also Eileen’s main carer. <br><br>Eileen was admitted to hospital with a suspected chest infection and increased dyspnoea at rest. </div>]]></description>

         <description><![CDATA[<div>George is a 63 year old man who lives at home with his wife who is his carer.  He is an ex-smoker and was diagnosed six months ago with liver cirrhosis due to alcohol abuse over a number of years.  The patient ’s condition has gradually deteriorated over the last six months and he has developed a severe ascites because of his cirrhosis. The patient has very good cognitive development however due to the cirrhosis and ascites he has developed increased dyspnoea and his mobility has now reduced. The patient has been admitted to hospital due to severe abdominal pain due to his ascites. <br><br></div>]]></description>
