There was a distrust for how psychiatry was being used, and how it was often more in the interests of society than in the interests of those with mental illness(es). There were different treatments (shock therapy, different drugs, different procedures). There was also much stigma against people with mental illness; they were seen as scary, less than human, and often had their rights taken away in mental asylums. In the earliest days of the project, Douglass attempted to contact director Milos Forman, who apparently underwent a mental breakdown while writing his film The Fireman's Ball, and refrained from leaving his apartment building for months. This movie had everything Douglass was looking for to turn OFOTCN into a motion-picture--setting, character design, etc. Kirk Douglass mailed Forman a copy of the original book  to begin working together--but the package never reached it's intended reciever, and thus Forman believed Douglass had broken his promise and resented him for it. It took roughly a decade to get the scandal sorted out.

The Asylum is ruled by  Nurse Ratched, a women who in herself does not have much strength, but her rule over the establishment is strict. Since she has absolute control, she can treat the staff and patients however she wishes; thus, blackmailing and unpleasant life is common. Nurse Ratched is focused on keeping the outside out and the inside in. When  McMurphy, the main character, shows up at the asylum, its isolation from the outside world is broken.  McMurphy offers a taste of the outside world and gives everyone and is "like a breath of fresh air." Since Nurse Ratched is set on keeping the outside out,  McMurphy is obviously a threat. It worked more quickly than the drugs we had then – more quickly than the drugs we have now – but it left me squeamish. The creators ran into many issues with wanting to change the ending and wanting to adapt it away from the book, which partly is the case for the final product but nearly as much as it was discussed during the creation. Some scenes were also shot without the actors knowledge; the actors lived in the ward while they were filming so conflict throughout that definitely arose. Forman and Nicholsen had conflict over what the film should be about because both of their interpretations from the book were different. Although there was conflict during the film, afterwards the movie had one of the longest runs in theatre in history. Huge, spirit-crushing state institutions – like the Oregon facility later depicted in the film – began reducing their excessive resident numbers and granting patients more rights.
It also catalysed the development of more effective anti-psychotic drugs that allowed more patients to be treated at home and live more normal lives.
Amid a minor revival in ECT in the early 1990s, The New York Times noted that thanks to the film's memorable images, "in the public mind 'shock therapy' has retained the tarnished image given it by Ken Kesey's novel: dangerous, inhumane and over-used".
Yet for many mental health professionals the book and film also had a negative effect. A 1983 study involving 146 university students found "considerable negative changes in attitude" towards people with mental health problems among those who had seen the film.
