On June 4 1996 Flight 501 of the ESA's Ariane 5 rocket exploded about 40 seconds into flight. The three satellites onboard were completely destroyed and engineers had no hope recovering or salvaging anything. The debris was scattered everywhere but no one was hurt.

On December 12 2002, Ariane 5 flight 157, the Ariane 5 exploded 3 minutes into the flight. This marks another loss of the Ariane 5 launch vehicle. This failure was another setback in the Ariane program. This made some people believe that the government could not trust the Ariane 5 as a suitable launch vehicle. The range safety officer killed the rocket after it plunged out of control into the Pacific Ocean.

Galileo launch

The 2 Galileo satellites were launched aboard a Soyuz rocket, was deemed successful at first. But after engineers analysed the telemetry data sent back from the twin spacecraft, there was something wrong. The two spacecraft, powered by the Fregat upper stage were locked in an orbit of 26,900 kilometers instead of the 29,900 kilometers expected, with an inclination of 49.8 degrees to the equator instead of the 55 degrees expected. This meant that the two satellites were useless and the ESA had probably just wasted a few hundred millions of dollars.

Indian Geostationary Satellite Launch Vehicle

The Indian based Geostationary launch vehicle exploded 47 seconds into flight at an altitude of 30 000 feet. This marked the second consecutive rocket failure for the Indian Space Program. Out of 7 launches, 4 have failed. This rocket is India's most powerful and can produce about 170,000 pounds of thrust per engine. This however makes the rocket really hard to control. During the flight, the booster nozzles were supposed to swivel to compensate for the tilting of a rocket. The command that mission control sent were not processed by the rocket and the rocket tilted over before destroying itself in a fireball.

Orbital Space Flight Company Antares experienced the second fatal space disaster within a week when an Antares launch vehicle, meant to carry supplies to the ISS. The craft experienced a major anomaly right after launch and exploded soon after. The explosion created a shock wave that was felt over a mile away.
