One of the biggest ways that I measure this is by seeing if they can teach a peer. If they can teach a peer the material, I know that they have learned it.

As for formal assessment, I think you can measure students' understanding when they create. When they are given the opportunity to share what they know (as opposed to choosing a, b, or c) I am able to assess what they know a lot easier.
