This project might benefits the younger age patients. As for elderly, we must educate their caregiver of the elderly patients or re-enforcement need to be done for them, on the important of self-administration of medications.

I’m currently working on a QI project that involves patient and caregiver for self-dressing on simple wounds. The project gives me more ideas in patients and their caregivers in their own healthcare process.

The project is a great initiative that empowers patients to manage their medication during hospitalization. It promotes independence and eases the transition to home. While screening and education are key challenges. And in the initial stage, nurses still need to check self-documentation to prevent errors, so it may increase nurse workload.
