The narrator is both insane and a calculated killer because he was able to kill the old-man without feeling any remorse, and he waited patiently, slowly filling up the old man's heart with fear.

you should have seen me. You should have seen how wisely I proceeded
— with what caution — with what foresight — with what dissimulation I
went to work!"
But also, in the duration of the short story, the narrator was constantly trying to convince the readers that he/she is a madman.
