If partnership can get through to directors feel they could get through to families 

CDSA uses DAYC2 for eval, includes SE domain but also rely on informed clinical opinion for things that can’t be captured in the standardized assessment 

DAYC does not correct for prematurity (but neither does the child outcome rating) – talk to families about how the child may look different than children born at term 

Child assessment done to collect information from multiple sources and inform IFSP – don’t use a specific tool but have questions for each developmental area they use  

Look for whether child has established condition that meets the eligibility criteria 

Referral form: has a place for concerns that can be written in 

Hospital uses a referral form that may include information about the condition 

Intake child history form delves into birth history, medical history 

Will let them know about CMARC, PAT, and other community resources – may not make formal referrals but will provide the info  
If close to 3 give them information about the school system so they can pursue screening  

Sometimes hard for families to add on outside services when they are getting a lot through the CDSA but let them know about resources 

School system lets families know about community resources including libraries, playgroups, preschools, etc.  

PAT does screening with first 90 days, then annually after that, will screen at any time if there’s a concern
