The first case is Mrs. A. On 20/7/2021, she got a needle in her hand. The reason was that she forgot to turn off the sewing machine while talking to the supervisor. Unfortunately, she pressed the foot controller when her hand was under the needle. At that time, first aid was given immediately in the sewing floor and then she was sent to the Medical center of our factory for further treatment. She continued her work on the next day. In order to minimize the cases like this, all workers need to pay attention to turn off all devices when not working for safety purposes.

The next person is Mrs. B. July 19, 2021. She burned her hand while she was ironing. She is a new worker so she has no experience and due to her negligence, she was injured. We gave first aid right at the sewing floor and she was then taken to our factory's Medical Center for further treatment. She continued her work the next day.

In conclusion, the data from this form is useful for possible improvements in working conditions. Managers need to provide training for worker, especially for the new staff to avoid unfortunate accidents which will affect productivity and work quality.

I have received the prototype and it has some different details between the spec sheet and the actual sample. In general, the prototype looks good as requirement. However, there are many details that need to be revised. The coat doesn’t have leather piping to cover label. Lack of black label with while letters. It should be 1” below collar. We need add leather piping to the label. Next, for the measurements, shoulder width is 4” wider, sleeves length is 2” longer and waist line is 6” smaller than the requirement in the spec sheet. We need take shoulder width up 4”, take sleeves length 2” and let waist line 6”. Besides, the color of the fabric is different from the spec sheet. The spec sheet requires 6 buttons but there are only 4 buttons in the sample. We should add as soon as possible. This is my comment about the actual sample
