         <description><![CDATA[<p>Exemplar: Agitated Patient Scenario<br>Digital Problem Identified:<br>Jessica was overwhelmed and didn’t document Mr. Anderson’s behavioral response in real time. This could lead to gaps in communication during shift handover.<br><br>Proposed Strategy:<br>I suggested using an automated mobile reminder system within the PointClickCare app that prompts real-time documentation every 15 minutes for BPSD-designated patients. The alert would guide PSWs to enter updates on behavior and interventions.<br><br>Why This Helps:<br>Timely reminders reduce documentation gaps, especially for new staff. A consistent digital prompt supports PSWs who are adjusting to workflow pressures and ensures critical updates are recorded and visible to the team.<br><br>Reflection:<br>In ARSnova, others suggested team-wide group chats or assigning a documentation buddy. I learned that digital solutions can also be collaborative. I plan to explore how our team might coordinate better using these tools.</p>]]></description>

         <description><![CDATA[<p>What Occurred in the Scenario:</p><p>Writer arrived to resident's unit and client found sitting on edge of bed, questioning writer's role and reason for entry to unit. Writer explained that writer is a PSW to assist resident with getting dressed this morning. Resident declined care, raising voice to writer that "I don't need your help." As per BPSD assessment, resident displaying signs of agitation, and verbal aggression. Writer consulted with management, who advised to play client's playlist of jazz music from his IPAD, particularly jazz music from the 1920's-1930's, as this assists in calming resident. Writer was provided code to access music playlist. Writer played client's music upon</p><p>return to room, with notable reduction in aggression and agitation. Resident then became agreeable to select his clothing for the day, and assisted to dress. Music was an effective strategy in reducing BPSD symptoms, and completing care. </p><p><br/></p><p>How Documentation Supports Communication, Safety, and Continuity of Care: This documentation supports communication as it records BPSD symptoms observed, to be able to better monitor common symptoms and ways to address or reduce those symptoms for resident, aiding in care delivery. It ensures additional staff are aware of symptoms observed, approach and interventions that were successful, to be able to implement those strategies in a timely manner. This promotes safety to staff and resident. It promotes continuity of care amongst staff to ensure they are aware of status and ensure they continue with care that is most effective for resident. However, this requires timely documentation to ensure staff coming on shift or within shift are aware in real time, issues that arose during shifts, symptoms of resident, and ways in which those were addressed to ensure continuity of care. It would be helpful for staff to have a point of care application or checklist that can be completed in real time, to prevent disruption or delays in continuity of care. </p>]]></description>
