I had a student that showed to program with a bruises on his arm, the staff asked what had happened to him and the student mentioned that his dad had grabbed his hard. I spoke to the staff and and the student, and we confirmed that it was true.

Next steps:  Ip is calling CPS and we provided the demographics of the student. Once we have more information we will keep you updated. We are working on a timeline

Student D.A. (#12345) 5th grade came to program with a large bruise on his arm. His IP asked him where he got it. Student said dad grabbed him hard.

Next steps:

SS will print out demographics.

L.A. told IP that she didn’t want to be here anymore. She stated that she sometimes cuts herself when she is alone. IP informed SC that she was holding her arm. SC is checking in with L.A. and ensuring she is not left unattended.

Counselor is off site so we will contact School Police for guidance. I will inform you of update from school police and begin a Timeline. Admin is on site and has been notified.

During snack, student L.D shares with IP that she doesn’t want to be here anymore, and she sometimes cuts herself when she is alone. IP states she looks upset and is holder her arm.

Next steps: counselor has been contacted. She will conduct the threat assessment. SC notified parents, and notified school police.

One of our IPs notified us that a student mentioned to her “ I don’t want to be here anymore. And sometimes I cut myself” and she looks upset and is holding her arm. We are with the student to ensure she is okay.

Next steps: we are currently looking for a counselor, and if we aren’t able to find one we will call school police. We will keep you updated.

L.B informed IP about not wanting to be here anymore and that the student cuts themselves when they are alone. Student is currently holding their arm. IP called SS for support to maintain visual on student. SS pulled student to speak to them.

Next steps: SS/SC will do fact finding. If admin/counselar are on campus we will contavt them, if not school police or Saraí will be contacted and student demographics will be gathered by SS/SC. SS/SC will update FS of new info.
