Is a process to evaluate any situation or item that may have potential to cause harm.

It is based on a combination of reactive, proactive and predictive methods of safety data collection .

Examples: -
Reactive: Safety Manager
Proactive: Hazard Register
Predictive: Data Analysis

b. Safety Risk Assessment & Mitigation

The identification, analysis and elimination of those hazards, as well as the subsequent risks, that threaten the viability of an organisation. It is a formal process within the Safety Management System composed of describing the system, identifying the hazards, assessing the risk, analysing the risk, and controlling the risk.

c. Training and Education 
                                        Develop & maintain safety training programme, ensuring personnel are well-trained and competent to perform SMS duties. 
 
eg, Incorporate training in SOP, Personnel to attend trainings

d. Safety Communication 
 
Ensure personnel are aware of the SMS to a degree commensurate with their positions, Conveys safety-critical information, Understand safety actions and procedures (introduced or changed) 

eg, Have regular briefings for SMS involving everyone within the management will be well-informed and updated, regardless of their position

Barriers to Organisational Learning:
a.    Barriers -  a circumstance or obstacle that keeps people or things apart or prevents communication or progress.
  1. Insufficient Reflection
  2. Insufficient Dissemination 
  3. Insufficient Action

b.    Symptoms - an indication of the existence of something, especially of an undesirable situation. 
  1. Denial of problems, incomplete or incorrect analysis
  2. Problem ignorance, solution ignorance, redundancy of effort 
  3. Lack of experimentation, implementation failures 

c.    Causes -  a person or thing that gives rise to an action, phenomenon, or condition, or  reasonable grounds for doing, thinking, or feeling something. 
  1. Performance pressure, competency traps, absence of learning forums.
  2. Intergroup boundaries, myth of uniqueness, narrow information bandwidths 
  3. Priority stress, bias towards activity versus results, perceived powerlessness 

d.    Solutions -  a means of solving a problem or dealing with a difficult situation.
  1. Stop and think, discuss and inquire, examine assumption and experience, seek insight, think in a new ways.
  2. Collaborate, hold learning conversations across groups and levels, promote exchange and adoption of ideas across boundaries, build effective systems to share, store, and retrieve knowledge 
  3. Invest in experimentation and risk taking, empower people to act on new knowledge, actively apply lessons learned, build a learning culture

Hazard Identification is the process where a service provider develops a method to identify the hazard that is related to the aviation product or services.

Example:  Before Operation Checks (BOC). Before we start with any operation, we need to perform Before Operation Checks to ensure that there are no hazards before commencement of operation. We also need to check personal equipment to mitigate any hazard that may occur.

2b: 
The service provider shall develop and maintain a process that ensures analysis, assessment and control of the safety risks associated with identified hazards.
