Pain score of 6-9 for past 8 hours.<br>Admitted for post-elective bilateral total knee arthroplasty<br>SD: Patient mentioned he vomited once after lunch and is more anxious about his pain.&nbsp;<br><br>Patient Problem: Joint pain related to post elective bilateral total knee arthroplasty evidenced by pain score of 6 to 9.<br><br>Goal: Mr Koh will be able to reduce his pain score to below 3 after 2 weeks.&nbsp;<br><br>Interventions:&nbsp;<br>-Prescribe pain medication to patient to relieve pain in knee.&nbsp;<br>-Apply cold patch to the incision on patient's knee to relieve pain.&nbsp;<br>-Prop up patient's leg when resting to relieve improve circulation to relieve pain<br><br>Evaluation: Mr Koh's knee pain will be largely reduced and his pain score will be below 3 after 2 weeks of care.</div>]]></description>

         <description><![CDATA[<div>Nursing Problem&nbsp;<br>Risk of Infection related to bilateral total knee arthroplasty as evidence by PCA of IV Morphine on demand and pain score of 6-9 over 10 hours.<br><br>Assessment<br>SD: pain score of 6-9 over 8 hours<br>OD: PCA of IV Morphine, vomitted and anxious about pain, temperature of 36.8 degree, pulse rate at 110 betas/min, respiratory rate at 22 breaths/min and blood pressure 146/88 mmHg<br><br>Goal: Reduce pain score from 6 to 1 within 24 hours. Relieve discomfort, access wound and change dressing if soaked or due. Maintain good condition of wound to prevent any infections by going for checkup regularly. Ensure that patient understands why he is having pain, and also communicate effectively<br><br>Intervention/ Implementation<br>- Elevate knees above waist level<br>- Change wound dressing when dressing soaked or due<br>- Make sure your surrounding and equipment are clean and sterile before using<br>- Morphine medication as prescribed to control patient's pain<br>- ordered and wound charting for record<br><br><br>Evaluation<br>- pain score will be reduce and patient will be comfortable&nbsp;<br>- vital signs will be on a stable side as patient is more calm<br>- wound infection will not occur as we’ve check the dressing daily</div>]]></description>
