         <description><![CDATA[<p>My 63-year-old male patient has a complicated medical history, which includes hypertension, diabetes, cardiovascular disease, and end-stage renal disease requiring hemodialysis. He missed several dialysis treatments, resulting in dangerously high potassium and phosphate levels, as well as a low sodium level requiring immediate medical attention. His medical history shows that he needs some major dietary restrictions because his potassium level of 6.5mEq/L could lead to life-threatening cardiac arrhythmias, therefore foods high in potassium need to be reduced. Also, because he has a high phosphorus level of 5.2mg/dL, limiting his phosphorus intake is extremely important as this may contribute to bone diseases and chronic kidney disease. My patient also has to manage salt because his sodium level is 130 mEq/L, which can result in high blood pressure.</p>]]></description>

         <description><![CDATA[<p>The diet restrictions I think are appropriate for this patient are Potassium, phosphorus sodium, and fluid. I mention this since his levels are high, yet the goal is to find balance for his health. I will reinforce the diet education that the RD may provide by asking my patients about their understanding of the diet, offering food preparation and substitutes, working closely with the RD, and reviewing the diet plan with the patient often.</p>]]></description>
