A: 1) "Hermaphrodite" is an outdated, offensive term for person who is trans. Instead we say trans now. 2) It depends on the reproductive parts they were born with. If they have a working uterus and ovaries, then it's possible to be pregnant.

A: It usually comes up in conversation. For example, if you're talking about someone and you say "her," someone might correct you with the proper pronoun. Apologize and move on, using the correct pronoun. If it's a certain situation where everyone is introducing themselves & their pronouns, you can do that, too. Or if you feel brave, you can just introduce your name and pronouns automatically no matter where you are when meeting new people -- this is often a norm if you know you're in a queer space.

A: Some get surgery, some don't. It's up to the person and how they feel. Some share their true gender identity with trusted people and their pronouns but don't alter their appearance at all -- or only sometimes by the clothing/items they wear. In general, surgery is expensive, so it depends on the type of surgery they are getting. I don't know enough about this myself, but I'm sure it varies with the doctor you go to.
