The trust helps the local community with many different services. In total there is around 260,000 people which use the services. Many of the staff could be blamed and at fault for the problems within the service, especially in certain departments. The responsibility cannot be forced onto an individual as the hospital is lead by a team of professionals who are all responsible for faults within the facility.  Within the four forms by Lawton and Rose Walsall Healthcare would have a managerial accountability. 

Managerial accountability is where the managers and leadership team are responsible for the faults of the establishment and also for how well the staff perform within their professional role. Within Walsall Healthcare Trust there is a lack of management and communication which means that the accountability will be on the management team. Lack of communication stems from there not being enough staff in the institution so the staff have to do double the work, also, as none of the reports were being recorded the blame would of been placed on managers as it shows lack of care and professionalism. The CQC report stated 'Ensure health records are completed appropriately and patient  data is confidentially managed. Patient confidentiality is maintained at all times across all services.' (Richards, 2015, page 4). Due to their being lack of staff there has been a lot of harmful incidents which could of been easily avoided such as falls. Another major problem which has been occurring in the Hospital is that they were failing to book people in for appointments, even after their administration system had been improved to make it easier. The report stated 'The trust employs 4370 staff. This includes 380 doctors and 1150 nurses.' If the Hospital is employing this amount of staff they shouldn't be struggling to meet the needs of the patients. 
With the issues from the staffing it shows public accountability as the responsibility and blame falls onto them if something goes wrong in the establishment. It also effects how the staff are working due to there not being enough staff in each ward. Also, staff were unable to even do simple tasks such as give patients appointments due to their new admin system. The leadership team has the duty and responsibility to ensure that the staff have all the knowledge and skill in order to work new equipment. The report also stated that incidents were not being reported and again the managers and leadership team should be aware of this and make sure the staff are reporting all incidents in order to be professional. The patients stated that the staff needed to be more responsive as if something serious was to happen the worry was that the staff wouldn't be there in time. - Training 
- Equipment
The hospital needed to make sure that they had the correct number of staff in each department and that they had all the correct training in order to be professional and meet the patients needs. The staff also needed to show more awareness about the patients and community.
