3/29/17 - Membership Forms by SMCC/Unity https://padlet.com/SMCC/djhyemv7kmcv Two things: 1) Though you've all been doing a pretty good job, there are still some membership applications that do not have emergency contact information. Please make sure we're being thorough each and every time we have new sign ups. If we don't have emergency contact info on hand during an emergency (something that recently happened), we could be facing serious issues. For the safety of our patrons, center, and ourselves, PLEASE don't leave the emergency contact blank. 2) Anyone under 18 cannot and should not be signing their own forms. I've come across a pile of youth membership forms that were signed by the youth themselves. This cannot happen anymore, please. Also, please note that if anyone is 14-16 years old, admittance to the gym cannot take place until s/he has been certified. Gina and Mele are in charge of this. I'll do better at streamlining info you should have had in the first place. For now, come to me if you're uncertain and I'll do my best to help.
