This checklist should be completed on admission and discharge for all women under the care of CGL and contains contact details for CGL and NNUH Substance Misuse Nurses who can be contacted for advice and support.

If a 28 week care plan has been completed by CGL and received by Skylark team, this will be will be attached to E3.

Please check E3 for Neonatal alerts and if there is a recommended plan for Neonatal Abstinence Syndrome (NAS) observations which should always be discussed antenatally with parents.
