Theaters in Shakespeare's time were ordinarily outdoors, with an open field containing the stage being surrounded by viewing areas three tiers tall. People payed different amounts to get better seating, the cheapest possible being standing. The stage itself was mostly bare, having only props directly nessisary to the plot of the play: Shakespeare depended more on vivid language than setting a literal scene.

Who may or may not have contributed. And as always if it was just him or others.

London Playhouses is basically just another way of saying a theater that was located in London. The main one was the one built by James Burbage just outside of London, as this is were Shakespeare had done many of his performances. IT was an open court that had a bare stage at one end. People would pay for tickets, varying at different price ranges, the cheapest had made you stand for the duration of the plays. This place however did burn down to the ground in 1599 when the roof caught on fire during a performance of Shakespeare's Henry VIII. Though a new globe was built in the same spot fairly quick, opening in 1614.

Back in the time of Shakespeare, the way they did theater was much different. For example, the stage was very bare compared to the present, only having props that are completely nessisary to the plot of the play. Also, male performers played both the roles for a male and a female, reflecting the sexist culture of the time. Costumes were a major part of the theater's investments, and while they were not always historically accurate to the play, they were usually very regal and beautiful.
