The accident occurred on 5th of December around 3.00 pm at Tesco Super Market, Small heath.
When are Where it happened
I am a check-out operator at Tesco. I was working on the till as usual, and everything was running smoothly. Suddenly, I heard a loud scream from the 3rd aisle. As soon as I heard the scream I hurried toward the aisle, I was shocked as I saw one of my college, Emma, laying on the floor.
Description
Emma could not move her legs and could not get up. Although she could not speak properly for her pain, she managed to communicate to me that she slipped on the wet floor while she was looking for baby food. When she slipped out few boxes fell on her leg as a consequence her right leg harts badly.

By the time my college Susan and Ava also have came there.
I asked Eva to get first aider and I call ambulance from my mobile. I was with her until the paramedic has came. After a time, the ambulance came and took her to hospital. I gave them my contact number and address for emergency contact .

After 5 hours Emma called me, and told she is alright but need to take rest.
Solution:
In my opinion to avoid a similar accident in the future would be to use a Wet Floor sign if the floor is wet. Also, designate one or two people who can use the ladder or clean the floor, provide them with appropriate safety training.

This report is aims to provide information about the current situation regarding sports Facilities in Birmingham and come up with the possible solutions to local council has to improving them
Facilities And Who are Used by
There are many sports and leisure facilities in Birmingham. For example we have 20 leisure centres, which are used by adults and teenagers, but this is not enough because this is a most crowded city in West Midland, where people need to mare outdoor leisure centres. In addition, we have six football grounds, which are used by young males and females this is not enough for such a large city because more women football teams have developed, who need more football grounds of their own. More over we have 20 children soft playground areas in different park, which should be enough. this is not the case because these playground are damaged and not been repaired for a while.

Therefore, the we suggest the following measures for the council:
1. Create 4 more outdoor leisure facilities in the city area for gentle and children.
2. Create 2 Football ground for women only.
Provide funding the maintenance of children soft play grounds.
I strongly support these solutions and I believe that if you implement these measures the sporting facilities in Birmingham will be the considerably improved, which will benefit everybody in the city.

This report aims to provide information about the current situation regarding sports facilities in Birmingham and come up with possible solutions for improving them.

What facilities and who are uses them?
Currently, Birmingham has about 10 leisure centres, which are used by adults and children. However, this is not enough because often you can not find a free place to register in any of these centres. In addition, we have six football grounds which are used by young males and females. This is not enough for such a large city because more women football teams have developed, who need one football ground of their own. Moreover, we have 20 children soft playground areas in each park, which should be enough. This is not the case because these playgrounds are damaged and have not be maintained for a while.

Therefore, we suggest the following measures for the council:
1.Providing funding for the maintenance of the playgrounds.
2.Build two ice rinks for skating and ice hockey for all age groups and all genders.
3.Create three more leisure centres in the city centre because this is a hugely population area.

I strongly these solutions and i believe that if you implement these measures the sporting facilities in Birmingham will be considerably improved, which will benefit everybody in the city.
