Manakala Mesyuarat Profesional KPM Bil.14/201 7 bertarikh 27 April 2017 telah meluluskan penjenamaan semula PST kepada Program Transformasi Sekolah 2025 (TS25).

Program TS25 merupakan sebahagian daripada usaha KPM ke arah meningkatkankemenjadian murid dan sekolah berkualiti dengan mengaplikasikan amalan terbaik dalam pelaksanaan pengurusan dan kepimpinan serta pedagogi dalam pengajaran dan pembelajaran (PdP) selari dengan hasrat yang terkandung dalam Pelan Pembangunan Pendidikan Malaysia (PPPM) 2013-2025.

Program TS25 bermula pada bulan Jun 2015 dengan kohort I sebanyak 99 buah sekolah. Seterusnya pada tahun 2017 sebanyak 100 buah sekolah dipilih untuk kohort 2 dan tahun 2018 sebanyak 401 buah sekolah dipilih untuk kohort 3, menjadikan jumlah keseluruhan yang terlibat adalah 600 buah sekolah.

Pada tahun 2019 pula sebanyak 500 buah sekolah telah dilibatkan dalam program ini.

Program TS25 melibatkan sokongan Master Trainer (MT) dan Rakan Educational Leadership and Instructional Team (Rakan ELIT) yang terdiri daripada 232 personel dalam kalangan pensyarah Institut Aminuddin Baki (IAB), Institut Pendidikan Guru Malaysia (IPGM), pegawai-pegawai Jabatan Pendidikan Negeri (JPN) dan pegawai-pegawai Pejabat Pendidikan Daerah (PPD).

Pada tahun 2017 hingga 2019, terdapat penambahan Rakan ELIT dalam kalangan Pensyarah IAB, Pensyarah IPG, S/Partners+ dan SISC+ berjumlah 1886 orang.

Menjelang tahun 2021pelaksanaan Program TS25 akan diurustadbir sepenuhnya oleh Jabatan Pendidikan Negeri (JPN) dengan didokong oleh Bahagian-Bahagian di Kementerian Pendidikan Malaysia (KPM).
