In 2020, 74 per cent of the global population had access to safely managed drinking water services, up from 70 per cent in 2015. Still, two billion people live without safely managed drinking water services, including 1.2 billion people lacking even a basic level of service, in 2020.

Between 2015 and 2020, the population with safely managed sanitation increased from 47 per cent to 54 per cent and the population with access to handwashing facilities with soap and water in the home increased from 67 per cent to 71 per cent. Rates of progress for these basic services would need to quadruple for universal coverage to be reached by 2030.

At the current rates of progress, 1.6 billion people will lack safely managed drinking water, 2.8 billion people will lack safely managed sanitation, and 1.9 billion people will lack basic hand hygiene facilities in 2030.

Eight out of 10 people who lack even basic drinking water service live in rural areas, and about half of them live in least developed countries (LDCs).

Water use efficiency worldwide rose from $17.4 per cubic metre in 2015 to $19.4 per cubic metre in 2019, a 12 per cent efficiency increase.

Assessment of rivers, lakes and aquifers in 97 countries in 2020 shows that 60 per cent of water bodies have good water quality. For at least 3 billion people, the quality of the water they rely upon is unknown owing to a lack of monitoring.

From 2015 to 2020, the population practising open defecation decreased by a third, from 739 million people to 494 million. The world is on track to eliminate open defecation by 2030.

Over the past 300 years, over 85 per cent of the planet’s wetlands have been lost, mainly through drainage and land conversion, with many remaining wetland areas degraded. Since 1970, 81 per cent of species dependent on inland wetlands have declined faster than those relying on other biomes, and an increasing number of these species are facing extinction.

Across the world, water stress levels remained safe at 18.6 per cent in 2019. However, Southern Asia and Central Asia registered high levels of water stress at over 75 per cent, whereas Northern Africa registered a critical water stress level of over 100 per cent. Since 2015, water stress levels have increased significantly in Western Asia and Northern Africa.

Data from 2017 and 2020 suggest only 32 countries have 90 per cent or more of their transboundary waters covered by cross-border cooperative arrangements.

Having clean water and sanitation means being able to avoid exposure to countless diseases. Every year, millions of people die from diseases caused by inadequate water supply, sanitation, and hygiene. Other than pneumonia, diarrhea is the main cause of death in children under age 5.

Key facts

Over 2 billion people live in water-stressed countries, which is expected to be exacerbated in some regions as result of climate change and population growth.

Globally, at least 2 billion people use a drinking water source contaminated with faeces. Microbial contamination of drinking-water as a result of contamination with faeces poses the greatest risk to drinking-water safety.

While the most important chemical risks in drinking water arise from arsenic, fluoride or nitrate, emerging contaminants such as pharmaceuticals, pesticides, per- and polyfluoroalkyl substances (PFASs) and microplastics generate public concern.

Safe and sufficient water facilitates the practice of hygiene, which is a key measure to prevent not only diarrhoeal diseases, but acute respiratory infections and numerous neglected tropical diseases.

Microbiologically contaminated drinking water can transmit diseases such as diarrhoea, cholera, dysentery, typhoid and polio and is estimated to cause 485 000 diarrhoeal deaths each year.

In 2020, 74% of the global population (5.8 billion people) used a safely managed drinking-water service – that is, one located on premises, available when needed, and free from contamination.

Overview

Safe and readily available water is important for public health, whether it is used for drinking, domestic use, food production or recreational purposes. Improved water supply and sanitation, and better management of water resources, can boost countries’ economic growth and can contribute greatly to poverty reduction.

In 2010, the UN General Assembly explicitly recognized the human right to water and sanitation. Everyone has the right to sufficient, continuous, safe, acceptable, physically accessible and affordable water for personal and domestic use.

Drinking-water services

Sustainable Development Goal target 6.1 calls for universal and equitable access to safe and affordable drinking water. The target is tracked with the indicator of “safely managed drinking water services” – drinking water from an improved water source that is located on premises, available when needed, and free from faecal and priority chemical contamination.

In 2020, 5.8 billion people used safely managed drinking-water services – that is, they used improved water sources located on premises, available when needed, and free from contamination. The remaining 2 billion people without safely managed services in 2020 included:

1.2 billion people with basic services, meaning an improved water source located within a round trip of 30 minutes;

282 million people with limited services, or an improved water source requiring more than 30 minutes to collect water;

368 million people taking water from unprotected wells and springs; and

122 million people collecting untreated surface water from lakes, ponds, rivers and streams.

Sharp geographic, sociocultural and economic inequalities persist, not only between rural and urban areas but also in towns and cities where people living in low-income, informal or illegal settlements usually have less access to improved sources of drinking-water than other residents.

Water and health

Contaminated water and poor sanitation are linked to transmission of diseases such as cholera, diarrhoea, dysentery, hepatitis A, typhoid and polio. Absent, inadequate, or inappropriately managed water and sanitation services expose individuals to preventable health risks. This is particularly the case in health care facilities where both patients and staff are placed at additional risk of infection and disease when water, sanitation and hygiene services are lacking. Globally, 15% of patients develop an infection during a hospital stay, with the proportion much greater in low-income countries.

Inadequate management of urban, industrial and agricultural wastewater means the drinking-water of hundreds of millions of people is dangerously contaminated or chemically polluted. Natural presence of chemicals, particularly in groundwater, can also be of health significance, including arsenic and fluoride, while other chemicals, such as lead, may be elevated in drinking-water as a result of leaching from water supply components in contact with drinking-water.

Some 829 000 people are estimated to die each year from diarrhoea as a result of unsafe drinking-water, sanitation and hand hygiene. Yet diarrhoea is largely preventable, and the deaths of 297 000 children aged under 5 years could be avoided each year if these risk factors were addressed. Where water is not readily available, people may decide handwashing is not a priority, thereby adding to the likelihood of diarrhoea and other diseases.

Diarrhoea is the most widely known disease linked to contaminated food and water but there are other hazards. In 2017, over 220 million people required preventative treatment for schistosomiasis – an acute and chronic disease caused by parasitic worms contracted through exposure to infested water.
In many parts of the world, insects that live or breed in water carry and transmit diseases such as dengue fever. Some of these insects, known as vectors, breed in clean, rather than dirty water, and household drinking water containers can serve as breeding grounds. The simple intervention of covering water storage containers can reduce vector breeding and may also reduce faecal contamination of water at the household level.

Economic and social effects

When water comes from improved and more accessible sources, people spend less time and effort physically collecting it, meaning they can be productive in other ways. This can also result in greater personal safety and reducing musculoskeletal disorders by reducing the need to make long or risky journeys to collect and carry water. Better water sources also mean less expenditure on health, as people are less likely to fall ill and incur medical costs and are better able to remain economically productive.

With children particularly at risk from water-related diseases, access to improved sources of water can result in better health, and therefore better school attendance, with positive longer-term consequences for their lives.

Challenges

Historical rates of progress would need to double for the world to achieve universal coverage with basic drinking water services by 2030. To achieve universal safely managed services, rates would need to quadruple. Climate change, increasing water scarcity, population growth, demographic changes and urbanization already pose challenges for water supply systems. Over 2 billion people live in water-stressed countries, which is expected to be exacerbated in some regions as result of climate change and population growth. Re-use of wastewater to recover water, nutrients or energy is becoming an important strategy. Increasingly countries are using wastewater for irrigation; in developing countries this represents 7% of irrigated land. While this practice if done inappropriately poses health risks, safe management of wastewater can yield multiple benefits, including increased food production.

Options for water sources used for drinking-water and irrigation will continue to evolve, with an increasing reliance on groundwater and alternative sources, including wastewater. Climate change will lead to greater fluctuations in harvested rainwater. Management of all water resources will need to be improved to ensure provision and quality.

What’s the goal here?
To ensure access to safe water sources and sanitation for all.
Why?
Access to water, sanitation and hygiene is a human right, yet billions are still faced with daily chal- lenges accessing even the most basic of services.
Around 1.8 billion peo- ple globally use a source
of drinking water that
is fecally contaminated. Some 2.4 billion people lack access to basic san- itation services, such as toilets or latrines. Water scarcity affects more than 40 per cent of the global population and is projected to rise. More than 80 per cent of wastewater result- ing from human activities
is discharged into rivers
or sea without any treat- ment, leading to pollution.
© UN Photo / Kibae Park

What are the effects of this?
Water and sanitation- related diseases remain among the major causes of death in children under five; more than 800 children die every day from diarrhoeal diseases linked to poor hygiene.
Proper water and sanita- tion is a key foundation for achieving the Sustainable Development Goals, including good health
and gender equality.
By managing our water sustainably, we are also able to better manage our pro- duction of food and energy and contribute to decent work and economic growth. Moreover, we can preserve our water ecosystems, their biodiversity, and take action on climate change.
What would it cost to correct the problem?
A study by the World Bank Group, UNICEF
and the World Health Organization estimates that extending basic water and sanitation services to the unserved would cost US$28.4 billion per year from 2015 to 2030, or
0.10 per cent of the global
product of the 140 coun- tries included in its study.
What would it cost if we don’t correct the problem?
The costs are huge– both for people and for the economy.
Worldwide, more than 2 million people die every year from diarrhoeal dis- eases. Poor hygiene and unsafe water are respon- sible for nearly 90 per cent of these deaths and mostly affect children.
The economic impact
of not investing in water and sanitation costs 4.3 per cent of sub-Saharan African GDP. The World Bank estimates that 6.4 per cent of India’s GDP is lost due to adverse eco- nomic impacts and costs of inadequate sanitation.
Without better infrastruc- ture and management, millions of people will continue to die every year and there will be further losses in biodiversity and ecosystem resilience,
undermining prosperity and efforts towards a more sustainable future.
What can we do?
Civil society organizations should work to keep gov- ernments accountable, invest in water research and development, and promote the inclusion of women, youth and indige- nous communities in water resources governance.
Generating awareness of these roles and turn- ing them into action will lead to win-win results and increased sustain- ability and integrity
for both human and ecological systems.
You can also get involved in the World Water Day and World Toilet Day campaigns that aim to provide informa- tion and inspiration to take action on hygiene issues.

The 21st century will be the century of water. At the beginning of the 20th century, there are " the 19th century for coal, the 20th century for oil, the 21st century for water & ot;. The 47th session of the General Assembly designated 22 March as & quo; World Water Day & quo; to the global shortage of fresh water resources. Globally, according to the United Nations, the global consumption of fresh water has increased by about 6-7 times compared with the early 20th century, and twice higher than the rate of population growth. At present, 1.4 billion people in the world lack safe and clean drinking water, that is, l of every 5 people are short of water. It is estimated that by 2025, nearly one-third of the world's population (2.3 billion) will be short of water, affecting more than 40 countries and regions, and China is one of them. China has been recognized by the United Nations as one of the world's 13 poorest countries in fresh water. Fresh water resources in China ranked sixth in the world, but per capita occupancy is only a quarter of the world average, ranked 109th in the world, and water resources in the time and regional distribution is uneven, there are 10 provinces, municipalities and autonomous regions of water resources is lower than the survival line, where the per capita water resources have less than 500 cubic meters. At present, there are 300 cities in China that are short of water, among which 110 cities are seriously short of water. They are mainly distributed in north China, northeast China, northwest China and coastal areas, and water has become the bottleneck of economic development in these areas. After 2010, China will enter a period of severe water shortage, and some experts estimate that China's water shortage will reach 60 billion cubic meters by 2030. Therefore, in order to ensure the sustainable development of China's economy, it is urgent to solve the problem of freshwater resources.
