Please collaborate with your nurse to determine MBHERP Team Members. There must be at least one clinician on the team.

Three (3) MBHERP Drills are required yearly:

Drill #1 by the end of October

Drill #2 by the end of February

Drill # 3 by the end of May

One drill must be anaphylaxis, and one must be a behavioral health drill.

Notify school nurse or nursing administration if errors found on this plan.

A team of nursing assistants will perform vision screenings for preschool through grade 5, grade 7, and grade 10. They will also perform hearing screenings on kindergarten through grade 3, grade 7, and grade 10.

We can screen young students under the age of 6 with an advanced vision machine called SPOT. To screen as many young students as possible, the nursing assistants will be coming to the schools to perform kindergarten vision screenings in September.

At the start of the school year and before screenings begin, nursing administration will send out an all call through Parent Square with the following message:

Welcome back! 

The LPS Vision and Hearing Screening Program as required by the Massachusetts Department of Public Health will begin next week and continue through the new year.

• Vision screening is done in grades preschool through grade 5, grade 7, & grade 10. 

• Hearing screening is done in kindergarten through grade 3, grade 7, & grade 10.

Best practice is for individual schools to send out a second reminder message with specific dates once screening is scheduled.

For vision screening, the testers need a wall and 10 feet to set up a wall chart.

For hearing screening, the testers need a quiet space without a lot of foot traffic or chaos. If not, the screening results will be inaccurate with an extremely high number of failures.

SEL will lead SBIRT screening.

CASA, LEHS & LCHS SBIRT will be done by Youth Wellness Coaches for Grade 09.

All other secondary schools will screen grade 07 & 10.

The school nurse and/or assistant will do these screenings for GR 1, 4, 7, & 10.

The school nurse and/or assistant will conduct these for all students in GR 5-9.

This authorization form has been approved by the legal department and includes all required information per the law.

All nurse's offices need to be locked when not occupied by a nurse. If staff are staying with students after school or at times when there is not a nurse, they will have to retrieve any emergency medication or have an alternative plan to open the nurse's office such as by the principal, custodian, or other staff on site. Please consult your school nurse.
