She was dropped by her foster mother which caused a head injury.
In April, Giller was asked not to see Skyla due to her troubled personal history. Her biological mother said. "I was given false hope by social workers who indicated she would be able to come home at some point.Giller said she had contested a social services application to send Skyla into care but a judge ruled against her.The type of abuse Skyla may have experienced would be neglect as the mother did not care well for her and was negligent. She had a swollen black eye and swelling to her head. The social workers did not inform Sarah Gillar the biological mother about Skyla's injuries. Skyla had an emergency brain operation, however in the morning her life support machine was turned off by doctors. Physical abuse is also likely as Skyla's black eye would not be explained by her mother dropping her.

http://www.whistleblower.co.uk/success-stories/foster-bay-death

http://www.ibtimes.com/foster-mother-allegedly-drops-baby-10-month-old-dies-after-emergency-brain-surgery-2585966

There were many missed oppirtunities for Victoria Climbé. Mainly starting with the fact that doctors put her scars down to ‘scabies’. The doctors and nurses who saw victoria for her injuries noted down “no child protection issues” even though suspicions had been thought of. Kouao and Victoria often went to ealing social services , due to ‘financial issues’. Another missed opportunity is when Victoria’s social worker stated that she was ‘unconcerned’ with Victoria’s care , due to Kouao having a part time job. Victoria was admitted into hospital for the second time for burns , but doctors stated there was so immediate suspicion of abuse , as she was treated for burns. Health visitors visited often , and so did social workers , so t raises the question of how was Victoria Climbés condition hidden so well?

Victoria was often tied up in the bathroom , and was given her food on a plastic plate. The small portions were enough to keep her alive for the time being. Kouao’s boyfriend often forced Victoria to sleep in a bin liner in the bath. Kouao reported that Manning had sexually assaulted Victoria on the 24th February 2000, bit withdrawed her statement the next day. Victoria was rushed to hospital on 25th February 2000 , due to severe malnutrition and hypothermia. She was later trasferred to an intensive care ward at St Marys in West London. Unfortunately , Victoria Died at 3.15pm the same day. Pathologists who examined her body found 128 different scars and injuries, most of them cigarette burns.

Daniel Pelka

In 2005, Pelk’s parents came to the UK, hoping for a better life and to find enployment.Daniel’s father , before he is born , was often convicted for drink driving and other driving convictions. Early 2010, Daniel and his parents moved into a flat in Bedworth. Early 2011, Daniel is admitted into hospital with a severely broken arm and is kept in hospital for at least 12 hours. He often complained about the pain of his bones grinding together in his arm. Luckzac ( mother ) and her partner tell Social services and police the injury was an accident , due to Daniel falling off the sofa and falling on his arm rather hard. At this time, Daniel’s weight was recorded to be 14.8kg. A few days later , Daniel has surgery on his arm. One missed oppurtunity here , is hat Social workers later closed the case a few days after , due to ‘medical evidence’ being reviewed. In March, Luckzac and her partner move into a three bedroom property. Daniel was often ‘imprisoned’ in a small box room and Physically assaulted. Luckzac had a visit from a health professional , who investigated her home unrelated to Daniel. However , she stated that her partner had tried to strangle her due to injuries on her neck. A few months later in August , luckzac told a Healh visitor that her son was struggling with his appetite , and was referred to a nurse in his school. Evidence shows that in 2011, Luckzac and her partner were exchanging text messages often about Daniel being ‘impisoned’ and spoke about starvation also. A health visitor understood that Daniel was eating excessively in the night and he was still stuggling with eating. He was referred to a paediatrician. Daniel would often steal food from school and sometimes home , and a teacher noticed bruises around his neck later in the year that looked like pressure marks. He came in to school a few days later with what looked like two black eyes. This is physical abuse , but nothing was reported ,and was essentially a missed oppurtunity. Daniel’s weight had dropped by 1kg , and after seeing a doctor , the doctor missed the opportunity to seeing abuse. The doctor stated that Daniel was fine and it was not a cause for concern. In early 2012, teachers stated that daniel looked pasty and his eyes often looked tired and sunken.... he was later spotted eating a pancake covered in dirt. March 1st 2012: Daniel was seen to take a half eaten bit of fruit out of a bin and eat it. Why didn't anyone see this as abuse? Luckzac collected Daniel from school and was seen on CCTV. Daniel was beaten fatally, mostly around the head. He suffered a fatal brain injury and was ‘left to die’ in the box room in which he was imprisoned. The same night, both adults search patient care in a coma. The next day in the early hours of the morning, a 999 call is made and the operator is told her son is no longer breathing and was compaining of chest pains. Daniel is pronounced dead the same day. Pathologists found at least 10 bruises on daniels back and shoulders , his hair was very thin and he looked emaciated. (www.telegraph.co.uk). The police or social services were not called and both adults left the hospital, later to tell Daniel’s headteacher he had suffered Heart failure. On March 5th 2012, a meeting consisting of Police and other agencies occured. After reviewing the body, it is clear that Daniel was fatally beaten and murdered and oppurtunities to see abuse should have been reported earlier and daniel could have been saved.
