         <description><![CDATA[<div>What a wonderful program!&nbsp; I can't believe they have only been running since October.&nbsp; They have served so many families in that short amount of time.&nbsp; The services they help link families to seem to cover all of the capitals.<br>I look forward to sharing resources with them in the future and learning from them.  I liked her suggestion of meeting over lunch some time.<br>I will be sure to share BOTK resources like the calendar and activity sheets with them.</div>]]></description>

         <description><![CDATA[<div>The 3 Resource Navigators at Children's Hospital and Clinic provide human, social, financial, food, energy and community capital for outpatient families with children. Through a carefully constructed referral system that begins with a mini needs assessment during well-child visits, the Navigators assist families with identifying their needs, help connect them with resources, and perhaps most importantly, follow-up with families to ensure that the services were appropriately matched to their needs. The Navigators focus on providing services in six areas: Food, Transportation, Education, Income/Employment, Housing and Immigration/other. Interestingly, one of the persistently challenging needs they encounter is helping families locate affordable housing. This aligns with one of Frogtown-Summit-University ELC's focus areas. I look forward to continued contact with Children's Hospital staff and look forward to the opportunities that our newly formed partnership will produce!</div>]]></description>

         <description><![CDATA[<div>Children's Hospitals of Minnesota's Community Connect has three steps:<br><strong>1. Identify<br>2. Connect<br>3. Follow Up</strong><br>These three steps provide the pathway to supporting human, social, physical, financial, environmental, cultural, food/energy/community capital that build a fair/head/healthy/safe/moral start with the families they serve.&nbsp; They hit them all as needed by families.<br>They talked about <strong>"warm-connections</strong>" that help the process go smoother.&nbsp; That's a lingo that we can include in our work...since so much of what we do is warm connections.<br>It's fascinating to learn about the <strong>NowPow </strong>database and it's availability to community based organizations.&nbsp; I hope to pursue that in our ELC and to pass that onto others.<br>The <strong>defined customized measures of success</strong> is another window to look through.&nbsp; Their scale is informative and helpful.</div>]]></description>

         <description><![CDATA[<div>It was a great meeting and hope we have an opportunity to meet again and share notes. I found our conversation very helpful and insightful. It was great to see a team that is just developing and some what defining their identity. I found interesting how much automation (NowPow) there is in everything we do including our work with and within the our communities.  But I really liked seeing that a personal connection (warm hand-off still have value.</div>]]></description>
