This is on the basis that pharmacists comply with the PGD, PCCS:I, and the additional guidance set out in this form. 


Appliations must be approved by the Health Board before any vaccinations are administered.

Please submit orders by 4:00pm on a Monday. Approved orders will normally be delivered the following Tuesday (approximately five working days later). 

All additional vaccine requests will be reviewed by the Community Pharmacy Team. Requests should be proportionate to anticipated demand, and existing stock should be, or be expected to be, utilised before additional supplies are requested. 

As part of the review process, WIS stock and refrigerator temperature records will also be reviewed, and you will be asked to update any outstanding records before an order can be approved. 

If vaccine is required urgently, please email CTM.CommunityPharmacy@wales.nhs.uk, marking your email URGENT and providing the reason for your request.
