On the outside, ghouls seem like other humans, but they have "kagune," which is like a physical representation of "the beast inside." The reason Kaneki himself embodies this trait of gothic storytelling is because he starts as a normal, kind, and timid character, even after the accident which make him a half ghoul with an extremely powerful kagune. This is because realistically, this much tragedy isn't as likely or common to happen, but it happens very often to people in this anime. Visually, an example would be a character being stabbed with a knife and after a couple seconds, there might be a pool of blood bigger than them, but then they get up again and walk around as if nothing happened. Contrastingly, in real life no one would bleed that much and live, and they wouldn't bleed that much anyway from one small stab wound. Because of this degree of exaggeratedness, the seriousness of the moment is removed and instead you focus on how the moment instead feels overdramatic to a degree.
