Females athletes have a higher chance of receiving an ACL injury. This is because the female pelvis is broader than a male pelvis, which alters the way the thigh bone, tibia, and femur work together. Because of this it places more strain on the soft tissues that help stabilize the joints.

An ACL injury affects several physical processes. It compromises knee stability, making it difficult to pivot, jump, or run. Swelling and inflammation often occur immediately, limiting the joint’s range of motion. Muscle strength and coordination in the injured leg may decrease due to reduced activity or disuse. If left untreated, the injury can lead to joint degeneration, such as osteoarthritis. Recovery processes, including physical therapy and possible surgical intervention, focus on restoring strength, stability, and function to the knee.

Magnetic resonance imaging (MRI) is the primary study used to diagnose ACL injuries in the US. An MRI uses strong magnetic fields and radio waves to create images of organs, tissues, and structures without the need for surgery or radiation (unlike X-rays or CT scans). Along with detecting ACL harm, MRI's can also identify concomitant meniscal injury, collateral ligament tear, and bone contusions. Before someone gets an MRI, evaluation of the ACL should be performed immediately after an injury if possible. However, this can often be limited by swelling and pain. When performed properly, a complete knee examination is more than 80 percent sensitive for an ACL injury.

The Lachman test is a common way to diagnose an ACL injury. The patient lies on their back with their knee bent around 20-30 degrees. The doctor holds the femur steady with one hand and uses the other to pull the tibia forward. If the tibia moves too far forward, it’s a sign that the ACL might be torn. It’s very effective in acute injuries, where it’s more sensitive than other tests. Depending on how much the tibia moves, the injury can be graded: Grade 1 for slight movement (mild injury), Grade 2 for moderate movement (partial tear), and Grade 3 for a lot of movement (complete rupture).

RICE (Rest, Ice, Compression, and Elevation) is the main treatment for an ACL injury. Rest is important, avoiding putting weight on the knee to prevent further damage, and using crutches or a knee brace can make things easier. Applying ice for 20-30 minutes every couple of hours during the first 48 hours helps reduce pain and swelling. Compression with an elastic bandage can help control swelling. Elevation; keep the knee elevated above heart level when resting to help drain fluid and reduce swelling. This treatment works best in the first few days after the injury.

For a complete tear of the ACL, reconstruction surgery is generally scheduled between 3 to 6 weeks after the injury occurs. This allows inflammation in the area to reduce and allows time for physical therapy sessions to focus on restoring normal knee movement. If surgery is performed too early patients may develop a scarring response called arthrofibrosis, which leads to stiffness of the knee joint. Delaying surgery beyond three months increases the risk of developing irreparable cartilage damage or meniscus injuries because of instability in the knee. The surgery focuses on replacing the torn ligament with a tissue graft to mimic the natural ACL.

A common long-term treatment for ACL injuries is rehabilitation, which uses physical therapy to strengthen the knee and enhance stability. The cost of a comprehensive rehab program typically falls between $1,000 and $5,000, depending on how long and how often the sessions are. Insurance usually covers rehab, especially when it's part of a post-surgery recovery plan.

Maintaining knee health long term after an ACL injury is essential to avoid other issues like osteoarthritis. This involves continuing to strengthen the muscles surrounding the knee, particularly the calves, hamstrings, and quadriceps. Also they should avoid high impact sports. Maintaining a healthy weight can further help with lessening the joint strain, and wearing a knee brace during physical activity can help some people avoid re injury. Long term support for overall knee function can be achieved by continuing to be active with low impact exercises and by continuously concentrating on muscle strength and flexibility. There should not be a specific cost associated with this type of treatment.

The cost of ACL surgery in the United States can range from $9,504 to $26,137, with the national average being $15,445. The exact cost depends on several factors, including the type of surgery, insurance coverage, and where the surgery is performed. Most health insurance plans cover ACL surgery when deemed medically necessary by a doctor, meaning you will likely have a significant portion of the cost covered by your insurance, although out-of-pocket costs like deductibles and copays may still apply depending on your specific plan. If you wait too long to treat a torn ACL, you're at risk of developing chronic knee pain and knee instability. Waiting to get treatment may mean that surgery is your only option. Or you may need to cope with ongoing instability by modifying your activities and giving up intensive sports.

After an ACL injury, a person could have difficulty with knee stability and coordination. Proprioceptive training involves exercises like balance boards, single leg stands, or agility drills that would help improve the persons ability to maintain balance and respond to movements. The key point of all of these exercises is to help reduce the risk of reinjury and could also help with a person's neuromuscular control. A physical therapist would be needed to help train the individual on how to do these exercises.

Altered proprioception, impaired motor control and muscle recruitment, as well as heightened reliance on visual feedback, can significantly increase the risk of re-injury and impede return to sport. Neuroplasticity, the brain’s ability to form new neural connections, plays a crucial role in recovery from ACL injury. This process can involve modification of cognitive strategies, recruitment of new and different neural networks, or adjusting the strength of existing connections in brain areas in charge of carrying out particular tasks. Short-term affects consist of improvement of function as well as prepare athletes for returning to sports. A long-term affect would be reduce of re-injury and fully returning back to sport. Therapists and psychologist can provide this treatment.

This procedure is aimed at treating the injury and restoring knee function. Some negative aspects of this treatment may be that it even though it is less invasive than traditional surgery, arthroscopic surgery still carries risks such as infection, blood clots, or damage to surrounding tissues. Recovery requires physical therapy, and not all patients may fully regain their pre injury knee function.
