Students don't ask questions because they fear that they would say something wrong and they fear that they would be laughed at if they say something fool. Most important is that most students see teachers as authorities, so they fear they would be punished or be critized by teachers for not understand an easy knowledge point. They may think that it's fine that I can figure it out later but end with nothing gained.

To be honest,I think the first is students maybe can’t express what they want properly.Then,sometimes it is difficult to interrupt the speaking when the teacher is lecturing and you have questions, most people will put them aside to ask later, and then they tend to end up not asking at all.
