Totoro is the god of 💀 and in the scene where Mei goes missing and her sandel is found at the pond it means that she drowned later during that scene Satsuki is asked if it’s Mei’s and can’t face the hard truth that it’s Mei’s and that she drowned 💀 after that scene she goes and desperately looks for her dead sister and calls for Totoro eventually opening the realm of 💀 meaning Satsuki dies as well she eventually finds her 💀 sister. In the ending scene where they go to the hospital their mom says “I swear for a second I saw Mei and Satsuki laughing up in that tree” and also in that scene they both have no shadows meaning their mother will also die soon 💀 in conclusion Totoro is the god of death and three people die 💀
