Social determinants of health

According to WHO, social determinants of health are the factors that affect health outcomes that are not medically related. It can be due to education, unemployment, housing, access to affordable health services, working conditions, food insecurity, social inclusion and non-discrimination, structural conflict, income and social protection, and early childhood development. Social determinants of health have a significant influence on health inequities. The lower the socio-economic position, the worse the health.

• One determinant of health that can affect refugees/migrants is social inclusion and non-discrimination in the community. In every community, people need to feel that they belong. It is known that social exclusion and discrimination are related. Discrimination through racism can influence a person's employment and access to healthcare, education, and services, which leads to social exclusion. Race and religion are the most common reasons for discrimination, especially for refugees from Muslim countries like Afghanistan and Iraq. Through the years, countless attacks, including harassment, slander, and assaults, have been reported. Thus, discrimination leads to poor physical and mental health and lessens life expectancy.

• Other determinants of health are unemployment and job insecurity. Most immigrants/refugees are from third-world countries seeking a better life. However, many migrants and refugees have difficulty seeking employment because of the language barrier, education, lack of qualified skills, and local employment experience. A perceived threat of job loss and unemployment-related worries can cause physical and mental health disparities. Hence, it becomes the risk factor for poor health outcomes of migrants/refugees.

• Access to affordable health services of decent quality is one of the issues that migrants and refugees in Australia face. Although Medicare is available for refugees, language difficulties, cultural differences, financial needs, legal barriers, and current government migration and settlement policy impact access to healthcare.

(Citation for the social determinants of health by WHO https://www.who.int/health-topics/social-determinants-of-health#tab=tab_1)
(Citation for the topics social inclusion and non-discrimination is from https://humanrights.gov.au/about/news/speeches/social-inclusion-and-human-rights-australia)
(Citation for the report of discrimination in refugees https://www.hrca.org.au/discrim%20and%20refugees.htm#:~:text=Asylum%20seekers%20and%20refugees%20in,and%20religion%20are%20clear%20enough.)
(Citation for the topic unemployment https://www.hcamag.com/au/specialisation/employment-law/migrant-and-refugee-workers-left-behind-on-job-employment.)
(Citation for the topic job insecurity https://www.ncbi.nlm.nih.gov/)
(Citation for access to healthcare https://pubmed.ncbi.nlm.nih.gov/15683352/#:~:text=These%20include%20language%20difficulties%2C%20financial,to%20health%20and%20health%20status.)

Domain 7: Professional

The purpose of this framework is for every clinician to be culturally aware of all aspects of their practice. For me, the most relevant part of competency standard 12.1 is being self-aware of my own culture and mindful of others. Avoiding biases, respecting their beliefs, and being professional when working with people from migrant and refugee backgrounds. Having learned that I now feel more capable of working with people from different cultures and backgrounds because I can provide care and understand them. This skill is essential to me in the workplace because I will encounter people with various cultures and health beliefs. To provide safe healthcare, I should always be aware of the culture and beliefs of my clients.

(Citation for competency standard framework https://culturaldiversityhealth.org.au/competency-standards-framework/)

The common health concerns in the Australian Refugee Health Practice Guide are Vitamin B12 deficiency, Malaria, and mental health issues.

Vitamin B12 deficiency manifests as macrocytic anemia. It is common with refugees from countries with food insecurity.
