It can also mean the patient has taken the wrong medication causing adverse effects, as well as the time can increase for a patient to see the next/correct health worker for their condition. It can also mean a patient can be diagnosed a lot later if a team was unable to provide adequate interprofessional practice. When it does fail, many issues, as explained, can occur that could have been prevented.
