OrganOx describes the metra is an organ preservation device used to maintain an environment that mimics a normal physiological state. The primary metabolic functions of the liver are maintained after an organ is harvested from a donor. The liver continues to produce bile, metabolizes glucose, and most importantly, maintains a normal pH.
Step 1: Retrieval- The metra has a sterile set that is primed with colloid solution and packed red blood cells.
Step 2: Transport- The organ is then transported to the location where the transplant team will be waiting.
Step 3: Transplant- The transplant surgeon is able to view hemodynamic markers, synthetic and metabolic performance of the organ throughout this period.

Normothermic Machine Perfusion (NMP) vs Static Cold Storage (SCS

Pros
The SCS method is the standard technique used for preserving a transplant liver between recovery and implantation. A technique Ravikumar et al. (2016) state is suitable for ideal organ donors. These researchers state using the SCS method is less suitable for organs that are considered 'high risk'. That is, organs that are donated after after cardiac death (DCD) where there is an increase in hypoxic injury or older donors with co-morbidities, among other high risk factors. Angelico et al. (2016) state these factors have a coorelation with graft post perfusion syndrome (PRS). PRS increases the risk graft injury, recipient morbidity and mortality. By using the new NMP technique with OrganOx metra, studies have reported to potentially have better effects on marginal organs, reduce ischemia-reprofusion liver damage, and PRS (Angelico et al., 2016). Another benefit of using OrganOx metra is the ability of the transplant surgeons to monitor the function of the organ before it is transplanted in the recipient (Ravikumar et al, 2016). OrganOx states the device has the capability of providing data from the transplant liver remotely via a smart phone, throughout the entire transport of the organ.
Cons
The NMP method has not been approved for use in the United States. A multi center RCT is in the works to test if the NMP method is superior to the traditional SCS method (Ravikumar et al., 2016) Angelico et al. (2016) also point out the first study to trial the OrganOx metra has significant contributions from the author's own institution. Ravikumar et al. (2016) stated "a high-risk graft remained 'high risk' even after NMP, but the physiological response after reperfusion was comparatively better"( p. 6). Even if the graft needed less vasoactive support through the activation, the graft was closely monitored for complications.

My Thoughts

I have worked for a Level 1 Trauma Center for 7 years now. In those years, I have cared for an endless amount of patients who in their tragic outcome, are able to give the gift of life through organ donation. Working fist hand with the organ procurement team and learning about this great innovation has now given me a new perspective. We have had many cases where donors were not in the healthiest of states and the organs donated are not accepted. I believe the metra device by OrganOx is an incredible opportunity to bridge the gap between the organs that are available for donation and the organs that make it to the recipients. This device can improve the outcome for the 'marginal' organs, yet have the possibility of extending the life of someone in need. I believe technology is forever changing and improving and it is amazing that we have the ability to help so many people with this device.

An achiever because they are known for working hard and are very productive. Carter with his great communication skills because communication is key to a good group relationship. Daisy because with her developing strength can grow talent in others. Pablo because of his positive motivation. It can inspire and lead others. Last but not least Randall for her strength in responsibility.
