The doctor image reflects where Rex demonstrates he doesn't like doctors and he doesn't trust them. Rex doesn't want any help from them even when his children are injured. He isn't satisfied that they could help their kids. Jeannette caught on fire because she was a three old who boiling sausages and her tutu accidentally caught on fire. Doctors helped her get better and watched her. Her brother cracked open his head and their parents didn't want to take him to the hospital and left him like that.
